It indicated kidney failure. Likely to need dialysis. However, many causes of kidney failure are treatable and a reading that high doesn't necessarily mean terrible news.

The doctors are staving off dialysis which I'm shocked by. Her function is down to 1-2%. I'm worried they're not being straight and actually they're just going to provide end of life.

She has cancer on top of kidney failure. She's been at this level of end stage kidney disease for two months with no significant intervention

What intervention are you expecting?
A creatinine level alone is not an indication for dialysis. It involves several factors, including whether such an invasive procedure is appropriate

She has 1% kidney function. Intervention must absolutely must happen.
