NHS 111- plan of action

Hi just wondered if anyone knew about this or had something similar. I have a medical condition meaning I need to go into hospital if I get a SBO (small bowel obstruction) - I've had several in the past. I asked the GP if they could put the info on NHS 111 for me. But it doesn't seem to have worked. When i ring 111 they can't find it. Previously they could, but I have moved surgery.

Does anyone know or have something similar. I guess need to contact GP about it again.

If you've moved surgeries it may not show anymore. We have an emergency care summary which details drugs but there is also a KIS (key information summary) which we have to get explicit permission from the patient to share with out of hours and I know when patients move to us we have to resend them because IT is so dated in the NHS.
ask your new GP if they can send this again. In Scotland where I am all the letters and results get taken from the document storage software which sits alongside the patient record and is populated into the document storage software at the new GPs but the actual written bit of your records does not repopulate the records software so all the diagnosis codes have to be re entered by someone. It's so clunky and "last century"

Hi Musical thanks. Yes- I told the new GP and they rang to tell me they had out it on again- but when I ring 111 they say they can't see it. But they said it may have been shared with urgent care / out of hours so am a bit confused. Previously they told me i had to say i have a 'plan of action' to 111. Today the woman said basically their system isn;t that good so maybe some kind of problem with that, and they basically follow what I would say to them in the situation. Bit they don;t tend to have info on previous history. With having recurrent SBOs that is pretty important as it is an urgent situation. I don't want to keep bothering the GPs if they say they have put it on again. But it would be reassuring to know what to do in that situation.

It should be fine if the plan isn't on there.

When you call you just say you have a 'particular plan of action' which has been given to you by a doctor. Then they will ask what the plan is and if necessary arrange an ambulance.

Particular course of action is a pathway in the triage system and bypasses the triage questions. It's really important that you are clear that this a plan given by a doctor as that is the only way assess that pathway.

Oh and don't worry about special notes they don't make a difference if you have plan in place. Just make sure that you tell them when they ask for the reason for the cal.

I feel a bit better, in the past I have found mixed times going into A and E especially at night, a GP mum I know said she also had the same thing and had to ring an anaesthetist she knew to rescue her from A and E one time. (the docs can take ages to assess if short staffed). So I prefer to go to nearest city hospital where my consultant is based, he said he will come see me and keep an eye on me if needed admitted again. But I'm a bit concerned as they won't know me there.

SBO is quite a common surgical problem though and in the past things have sometimes been more unusual so i should be OK. if i take the plan in.

Can you ask the GP for a copy of your action plan. Then if 111 can't find it or you have to go to a&e and they can't find you can just give/read them a copy?

Thanks, yes i have a copy. Also the surgeons have it in my hospital file as well as do the GPs. However it is dated 2015. I wonder whether to get to reviewed or not, nothing has changed though.

I did most of this myself btw. In case anyone similar want to do the same. It has things like pain meds which help me and a summary of past history for A and E.
