Please talk to me about pelvic pain

So far this year I've had 3 surgeries. First, to remove a 3cm polyp from my uterus. Second, a laparoscopy to look for and remove small endometriosis deposits. And third, a uterine ablation. My periods were heavy, clots and flooding. They are now 'normal ' in terms of blood loss, but the pain just keeps coming. I get bad cramps, ovary ovulation pain, and now my whole pelvis and down my thighs feels like it's being twisted and washed in acid. It's like heartburn pain, but in my lower abdomen. It makes me feel sick. I take paracetamol, ibuprofen, codeine, use heat pads and a tens machine. I'm due on and it's almost unbearable. I'm due back to see the consultant next week. Fuck this.

I get similar pelvic pain, although no where near as bad. I'm currently being looked at for adenomyosis which is a form of endometriosis but the cells have migrated to your uterine muscle. From my research the 2 often go hand in hand although it all seems to say if you have adenomyosis then you may well have endometriosis so I'm not sure if it works the other way too.

Have they given you ideas for treatment? From what I've been told if they find out I do have adenomyosis the best treatment is mirena coil or oral contraceptives.

I'm sure this is al stuff you know already, you are obviously much further along the process than I am!

I can't tolerate any format of hormonal contraception. I had hyperemesis in my pregnancies and since then I just vomit as soon as I start with the pill. I'm very reluctant to try the coil for the same reason. Tbh I'd happily have a hysterectomy if it meant an end to the pain.

Oh no, that's awful then. I don't react well to hormonal stuff either so dreading the suggestion.

Have they refused a hystorectomy then? My pain is not too bad at all but it's very draining being uncomfortable all the bloody time. I'm coming to terms with not having another DC and I'd rather they took the lot out while they can and I'm young and healthy enough to recover fairly quickly. I know they'll say no to me though!

Maybe everything has been irritated by the surgeries. I had major pelvic surgery this year and am still having pain/bloating 6 months on. It's a shock to the system and takes a long time to recover. I wasn't expecting it and didn't have it pre surgery!

Copper coil is hormone free if you're looking for contraceptive options.

I haven't been refused a hysterectomy, they're just trying everything else first. This is my second cycle post ablation, so waiting to see how the bleeding is. I'm worried the real problem is my ovaries. They won't take them out as I'm 37.

It may be endometriosis that is causing all this. My guess is that the lap was not thorough enough this time around either.

I would seek a second opinion from another gynae surgeon if you have no luck with the surgeon this week; preferably someone with a specialised interest in endometriosis as well.

If you had a hyst your ovaries would likely need to be removed at the same time if endometriosis is the underlying cause of your pelvic pain.

If you do not already do so I would keep a daily pain and symptom diary as this will also give the gynae clues.

If endometriosis is the cause as well I would think long and hard before undergoing such an op as such pain can recur post hyst.

Quick update. Consultant believes my endometriosis is causing the problems. I am on a 6 month course of injections to simulate menopause to see if the pain improves. Then, we'll see if it comes back. If it does, the only real option in my case will be full hysterectomy and ovary removal. A big decision. So, this seems like a good way forward for now.
