Vaginal vulval and urethral pain help please!

I have been experiencing these problems for a year now.
I have pain in my urethra that feels like a uti but isn't. I also have regular pain around the vulva and vaginal pain. This pain leaves me bed ridden sometimes and codeine doesn't really touch it.
I am on amitryptyline and sertraline. I use dermovate and dermol prescribed by the doctor at the gum clinic but no one knows the cause. I have been tested for everything and blood tests for hormone levels and thyroid. I was wondering of anyone has any ideas or can help.

Sorry you are struggling. You probably already know this, but it sounds like you may have a condition called "vulvodynia" - thought to be an oversensitivity of the nerve endings in the vulval area - there are a few specialists in this condition (usually gynaecologists) who may be able to go through all the treatment options with you - but as far as I know there is no easy solution to it.

I had this. It was bloody horrendous and wiped out a couple of years of my life. I was able to work but my social life died, I was in constant pain and sex, tampons, sitting were agony. I saw a private gynae who put me on a long term course of antibiotics (a fairly full on course of ciprofloxacin and metronidazole for a month followed by a prophylactic dose of nitrofurantoin). I was on antibiotics for several years, and also used dermovate when I had burning pain (I had intense intermittent UTI type-pain and lower grade constant burning in my vulva). The pain gradually improved and I am now pain free (and was able to have two babies vaginally, which I never thought I'd be able to do). This is not the normal course of treatment for vulvodynia (I never tested positive for bacteria so it's counter intuitive), but it did work for me. Some other things that helped a lot were:

Coming off the pill (ALL hormonal contraceptives made the pain worse for me).

Stopping wearing trousers, especially jeans. Cutting the crotch out of my tights (sounds weird, but they made me really sore!)

Making sure my vulva never came into contact with soap or shampoo

Wearing only 100% cotton underwear (from m&s) and washing it at 60 with only a smidgen of detergent

Avoiding drinking too much tea, coffee or alcohol and drinking masses of water to dilute my urine.

No-one really knows the causes of vulvodynia and I often wonder if it's really several different conditions lumped under one term. This means that different treatments work for different people, so you may have to try a few. Have you looked at the vulval pain society website? They also run a support group if you're near London.

Good luck, I know how hard it is to be in constant pain and have to hide it from everyone. You have my deepest sympathies.

One more thing- swap to plastic free pads and tampons (if you can use them). Natracare are good.

I have just ordered some reusable pads though until this week I was fine during my period! Then this time to comes on whilst I am on my periodHmm there seems to be no linking factors. I was with water and dermol I only drink herbal teas and water too, just so fed up

Thank you by the way Bolshy occasionally when I have been swabbed they have found pus cells or a slight imbalance of bacteria. My gum doctor is now going down the line of physio and counselling, and steriod cream but just doesn't seem right iykwim?

Can you push for another referral? I lost count of how many I saw, they all seemed to have their favourite treatments as well. One told me to come off the antibios as they wouldn't work. I tried it and immediately had a flare up of UTI type pain Confused so went back on them. Also tried ami but it made me so groggy I couldn't function at work and didn't really help my pain anyway.

Bizarrely, having kids made things better. Think everything got obliterated"reset" down there......

I would ask for swabs for obvious things even if they say you don't have them so that at least you've ruled things out officially- thrish, bacterial things in vagina. Ask about test for low grade infection where they culture urine for longer than the standard 3 days in lab. Go with a sample ready- a concentrated first morning urine sample. There's other things like ureaplasma wgich aren't tested as standard but they might know. The other things to ask about is trying other pain relieving things- different combinations etc. Good luck. Hope you get some answers
