thrombosed external hemorrhoid

Hello I'm 29weeks pregnant tomorrow and I joined mumsnet in need of help, I'm posting in desperation to see if anyone can help me, since Wednesday ive been in absolute agony with a very large purple and extremely painful thrombosed external hemorrhoid, I can't walk unless it's like a baboon, going up and down the stairs is torture and everytime I go to toilet even for a wee I'm just in tears the pain is so severe I compare it to the worst tooth abscess and infection I've ever had and I thought I had a high pain threshold but I'm just at my wits end.

I called the doctors Wednesday (they operate a call system) they prescribed me anasol cream, it did nothing by Thursday afternoon I was crawling the walls I called the doctors again same one answered he gave me ansol suppositories and latchlose I also read online (not knowing at this point it was thrombosed) that I could try gently pushing them back in, which I did and it gave a tiny bit of comfort but they were still protruding and came right back out the next time I went for a wee.

Thursday night I got abit more sleep as it had kept me up the previous 2 nights and Friday morning it felt somewhat less painful sitting on a side and walking to the kitchen etc but by Friday evening I was sobbing the pain had returned twofold and at midnight I just couldn't bare it anymore I called 111 and they said go straight to A&E which I did and unbelievably on a Friday night it was very quiet we were the only ones there! The walk from the car park was agony and sitting in the waiting room was impossible fortunately after 30 mins I was seen by a female doctor she attempted to do a rectum exam but it was too painful so she gave me 2 paracetamol and said she would get a surgeon

When the surgeon came to see me I said "are you going to take them away?" He said if he could he'd be a very rich and popular man :( he examined me and said basically they couldn't operate because I was pregnant and that 2 things can happen naturally; 1 they can sort of strangle themselves and die leaving a skin tab which can be removed after I give birth or secondly they shrink and resend or dissolve on their own, I asked if this would reoccur through my pregnancy and he said possibly :*( he told me by Monday I should be feeling better and gave me some numbing gel (which is useless)

today (Saturday) I have had a hot bath, used regular ice packs, made my own support pack by rolling up 2 sanitary towels (didn't think I would need them for a while!) and putting one front and back of the pile to aleviate the pressure, I've taken regular paracetamol and continued with the suppositories and latchlose - nothing is making a difference and the pain is honestly driving me demented is there anything anyone else can suggest that I take ? Or do? Or use? I feel like getting a knife and lancing it off myself!!! (I won't)

I've had a really rough pregnancy with hypermesis gravidarum for 26 weeks it was just easing (only bad until around 11am now) I've also got gestational diabetes (I'm a big big girl) and pregnancy insomnia and I just wanted to start enjoying my pregnancy so my mental state is also very low at the moment

I do have to say I have a brilliant husband and he's foing everything for me he's brilliant around the house etc

If anyone can offer any advice I would be very grateful

Thank you in advance

Good luck with the baby, sorry to hear the TEH recurred, I had it recur once or twice but never as bad as that first time when I stupidly let them slice it

Justyouwaitandsee · 28/04/2015 06:38

Did you ever try Proctosedyl? (As I suggested right at the start of the thread) I have recommended it several times on here, when doctors have said there are no alternative options and each time they have been willing to prescribe and the person has reported relief. As a veteran sufferer, it really is the only thing that helps me and takes the pain away...am desperately trying to avoid being in pain from them when going into labour.

Applecross · 28/04/2015 06:50

I had one after I had dd2 (and 4 other external ones and tears) it was honestly the worst bit of it all, undignified and excruciating, scheriproct cream and suppositories and ice packs (boots do a special ice thing you can use too) - I even took my cocodamol for this pain - it will get better soon, you ARE nearly there. I felt as though it never would but by 3-4 weeks post parting i could scale everything right back and now I just eat a lot of porridge :) Good luck with the delivery!!

Oops, missed that. Am sleep deprived and it's my due date today. Will take my evangelical preaching elsewhere, and hope others can offer some better suggestions! Hope you manage to get things sorted OP Flowers

Justyouwaitandsee don't worry it's fine I have ansol and Proctosedyl tmi but I'm using ansol internally and Proctosedyl externally it honestly hasn't got to the same level as before pain wise so perhaps this time it's working but last time nothing at all worked except 3-5 baths long soaks a day I still have a skin tab there the size of your little finger end but it's not painful, the new one is a complete separate one its smaller and feels lumpy and angry I can push it back in and do so several times a day :(

Stupidflanders bless you that sounds just like me I was distraught with pain at one point and considered self lancing many times it got so bad :/ I never thought about stitches I better ask them on Friday :(

I too am going to have to name change after this! The internet is perfect to get advice about things you want to forget about in real life - here's hoping they are soon a memory for you! x

Take a plastic rubber glove and fill the fingers with water and freeze. Remove the 'ice fingers' and insert. Should help with the swelling a little. Good luck Flowers

Adason43 · 29/04/2015 04:16

That's a good idea thank you I'm awake now as the pain has progressed to keeping me awake now :(

Oh, you have my sympathy, I have suffered throughout my life with bouts of piles. By far the worst was after I gave birth, 3 days after because I pushed for about 2 hours (yes they let me!) my bum just prolapsed. If I tell you I had so many prolapsed piles (8) they were so bad they were packed together in 2 rows and stood proud of my backside, I couldn't lay on my front (tender boobs) back or side, I could not get any sleep and it was worse than the childbirth pain. I had a ring to sit on (don't bother) and it took about 3 weeks for them to subside. The nurse thought I was being mardy until she looked and said she had only ever seen 1 other case as bad and that needed surgery. Since then I have had other bouts, one really bad time the GP tried to push it back and I nearly went through the ceiling with the pain so I won't ever try that again. I would say if you can get hold of lidocaine cream that numbs the area and also there's a good anusol spray that also numbs, I just put up with them until they finally shrink, something I have got used to but I can sympathize as the pain is just awful. Surgery is also not something i'd want it sounds bad ad they just come back anyway :( Hope they shrink soon x

Adason43 · 29/04/2015 10:02

Oh Sayreville I'm so sorry that's just terrible I don't know how you got through it you are incredibly brave x if these don't go after the birth I will insist on surgery because I'm not as brave as you and I can't cope being in constant pain my eyes water thinking of 8 !!! ??

OP posts:
sayerville · 29/04/2015 10:29

Adason I'm not brave by any means in fact quite the opposite as surgery make me so scared! Yes there were 8 so tightly packed they were square shaped - sorry TMI!! I am sure you'll feel better soon x

grimbletart · 29/04/2015 15:35

You poor people. Can I just say that if you are left with ongoing or recurring problems surgery really is the answer. I put up with recurring piles for many years and finally got to a surgeon who said that anything less than removing them surgically was a waste of time.

The surgery was absolutely OK (and this was many years ago when techniques were less advanced than today). Once I had got over the first eye-watering poo - I was scared it would bust the stitches - I recovered within a few days and not had any trouble since.

Please don't worry about having surgery - the thrombosed piles were far more trouble!

Adason43 · 23/05/2015 06:15

Hi just an update, typically things didn't go smoothly for me at the birth due to my GD the hospital booked me in for an induction for which I endured 4 days for all attempts to fail, anyhow in the end I couldn't go through another round and after going home for a 24 hour breather I opted for a CSection which was planned and really very good

So I had a little girl 7lb 11oz on the 6th May and she was worth every second of sickness and agony GrinGrinGrin

With regards to my problem I have 2 skin tabs which are still external but now not painful and when I strain a massive cluster of "normal" piles I asked the Dr about removal and they said it was too soon wait for my 6 week check up

TMI but at the mo I'm all over the place from constipation to diarrhea and nothing in between so hoping when all settles down I'll know if they will go of their own accord or if surgery is the next step

Thanks again for all your help and advise and if this thread helps anyone else then please remember bath bath bath it really works xxx

Congratulations on your little girl Adason. You sound as if you had (and are still having) a rough time. Hope things improve soon.
