Testicle pain! Help please

Hello

my partner is experiencing quite a lot of pain in his right testicle, it has been going on for about two hours.

there is no swelling or any sign of something wrong, he has not hit it or anything.

Any advice on what we should do? I have read about testicular torsion but I'm not sure if this is the problem as that says you usually see swelling and have abdominal pains with it?

Any experiences or advice??

Children usually have abdominal pain partly because they are poor at localising pain (also because of referred pain but that's irrelevant).

I think you need to speak to OOH, it may be nothing but it could be torsion which needs treating within 6 hours.

Get to A&E immediately.

He should not have continuous severe pain there for two hours.

Torsion can occur without obvious cause or reason.

Hi

We went to A&E and they said it wasn't Torsion, could possibly be an infection but no sign of infection in urine sample.

They said to take painkillers and take another sample to the GP on monday if it's not cleared up.

It hasn't cleared up yet, he's still in pain when the paracetamol wears off. He doesn't feel pain when urinating as they said he probably would if it was an infection.

Anyone experienced anything similar?? Quite confused without a diagnosis. He'll call the GP tomorrow and try and get an appt the same day.

Banned him from doing any heavy lifting or anything that might strain him in any way

Did they scan him?

I know ultrasound is not great for soft tissue injury stuff but seems to me like an MRI might be in order if it continues.

I have had epididymitis, which is an infection in the epididymis which is attached to the testicle. It does hurt a fair bit.

If this does not stop today or gets worse, or he has any swelling go back to A&E and demand a second opinion and scan immediately. It really should not be happening. It is not something that is 'just one of things'.

Hi - I've name changed back because he's not that embarrassed about it and I'm pretty anonymous on here anyway!

Thanks for your reply. I forgot what they said but that was it - epididymitis. He went to the GP today who said his urine was still clear but his temperature was raised and prescribed him antibiotics (for a month!) and said it might continue to hurt for 1-2 weeks whilst they start to work.

How long did your epididymitis take to clear up? How long should we wait before we bug for a scan?

Testicle pain is pain occurring in or around one or both testicles. Testicle pain includes a number of possible causes. A mans testicles are very sensitive organs, or even a minor injury can cause testicle discomfort or pain. Testicle pain may arise in the testicle itself or from the structure behind the testicle (epididymis). Testicle pain can also be caused by problems that start in the groin or lower abdomen.

Severe testicle pain which comes on quickly can be a manifestation of testicular torsion - a twisted testicle that may quickly lose its blood flow. This condition requires immediate treatment to prevent loss of the affected testicle.

moobaloo - it carried on for a quite a few weeks and has recurred a few times. Problem is that antibiotics don't really reach the area very well. It is a peripheral part of the body with poor blood supply.

I had to have a STD test because epididymitis is often associated with sexually transmitted disease. All very embarrassing as DW was heavily pregnant at the time. Our GP was insistent that for public health reasons it had to be tested so off me and DW went to the STD clinic.

Annoyingly I actually got the condition as a secondary infection in hospital as I was being treated for viral meningitis.

Has either of your DH's had a vasectomy ? Chronic epididymitis is a direct result of a vasectomy and it's as common as 1 in 10.

I've had it since my snip over 20 months ago. I've also had the left epididymis chopped out, but it's not helped.

It's exceptionally dificult to treat and very painfull. I'm hoping to get a nerve block done, but weather tha't possible is another thing. The surgeon won't remove my worse testicle as it can't be guaranteed to relieve the pain.

I'm currently on 30-60mg of codeine every night !

No he hasn't had a vasectomy. He got the initial infection five years ago. Antibiotics cleared it up within a week. About a year later, he got it again. It's never gone. He is under the pain clinic, who keep telling us they've never had anyone have it for more than ten years. I wish he were just on codeine. He is on much, much stronger stuff than that. I just keep hoping that he'll wake up one morning and it'll be gone. We looked into removal but the chances of improvement are less than 50K% so really not worth it. He's not had anything removed.

Mine started after the snip. We've tried the removing the epididymis but that's only got a 50% chance too. I've now been referred to the pain clinic, and I suspect they will try to put me on the ami or gabapentin, but I've refused in the past for other injuries - I find they make me dopey, which is not good for a 'mental' job.

May I ask what he is on ? I am going to try and push for a nerve block in my spermatic cord, but I doubt they will do it. I've asked about surgical de-nervation and they won't do it as it's too risky.

An epididymetcomy is probably not an option for your husband as it makes you sterile.

I only take the codeine at night so I can sleep, so put up with the pain during the day. Mornings are excruciating, with pain starting by about 5am and having to get up by 7 - weekends are a mare as I'm in too much pain, so no lie in. Takes over an hour to ease once up and about

I'm 55, so the sterile thing wouldn't actually be a problem. It's just not worth the risk unless the odds are increased to at least 75%.

He found the gabapentin/pregabalin route stopped him thinking, so we created a bit. He's on Tapentadol, it's a controlled drug, he takes slow acting and if he's going through an acute phase, he can take a fast acting on top. He also takes duloxetine, which apparently helps the tapentadol. It's a SNRI. supposed to fool the nerve endings somehow. It helps some of the time. We find stress and extremes of heat exacerbate things.

Blimey, I'm mid 40's and physically very active and I don't/won't be taking anything as strong as that as I could do myself an injury with strong pain killers.

It's a very difficult condition to treat - at least I know what caused mine (and the blundering GP who did the op). I found the only truth I got was at the 'hospital' stage - the GP's couldn't give a hoot.

He's 44 and was very active. He was given an antibiotic for the second infection, cipro. Cipro can create peripheral neuropathy. It can also create tendonitis. In every single joint. Dh uses two sticks to walk now. I wash him, dress him, etc.

My dh had similar testicle pain about 4 weeks ago. They gave him a months worth of antibiotics which of course he hasn't taken. They referred him for a scan too but we haven't had a date yet. What are they checking for with the scan? He hadn't had the snip but it sounds v similar.

If the pain has gone now do you think it's ok to just leave it with no follow up?

He's 54 and otherwise fit.

I would get it checked just in case. Epididimytis has a nasty habit of coming back.

dawndonnaagain, was it the Cipro that caused your partners mobility issues, or the drugs he is on ? That sounds horrendous

Scans are usually to check for nasties (cancer), cists or even the blood supply. Usually trying to rule out anything sinister.

They can also spot a swollen epididymis, but they are always swollen on men who have has the snip. They can also find the scar tissue development from the snip too.

We have been told in all likelihood, it was the Cipro both exacerbating his prostate and testicle problems and his joints. Whole damn thing is a nightmare.
