Ankle/foot injury plus fractured toe getting worse after 6 weeks? Lisfranc?

I injured my foot on holiday. Foot went down a hole, then twisted right over under me.

Usual pain and swelling, so rested, raised, iced etc, assumed it was a sprain and rested with it raised as much as possible for the rest of the holiday.

After a 17 hour journey home, during which time I couldn't rest my foot (VW camper - not aircraft) I was in agony and ended up going to A&E a couple of days later.

Was triaged and sent for xrays, with the NP saying she was almost sure it was fractured due to pain levels, swelling and the way I was/wasn't weight bearing.

Xrays came back with a fracture in little to, adjacent to the joint, but no other signs of fracture. As it had been two weeks, they wouldn't/couldn't do anything for the toe and as they couldn't see any other fractures I was sent home without any treatment, not even a support bandage - again with the NP saying she was really surprised it wasn't broken, as she would've put money on it. (She showed me the xrays in detail and the 5th metatarsal was definitely intact.)

Fortunately, my sister lent me some crutches and I have been strapping it up to support it, so that I can get around and raising it whenever possible - although I have 3 dcs to chase around after and that means 3-4 school runs a day. Fortunately I only live across the road and down a small close from the school.

It's now 6 weeks since I did it and the pain is just getting worse and worse. The ankle swelling has pretty much gone and the little toe is much better, but the pain along the outside midfoot area and under the sole/arch is unbearable and I still can't bear weight. Whereas the ankle swelling has all but gone, my foot itself is swollen over the top, right down to my toes and the whole foot regularly goes dark purple and really cold.

Yesterday I had to do 5 trips up and down to the school due to parent information evenings and I was in agony. By the time I finally got to the sofa I couldn't move my foot without screaming (and I promise I am no wuss - I generally have a high pain threshold) and I was too scared of the pain to move until dh helped me to bed.

After yesterday I made a GP appointment for first thing today. Went this morning, but after 13 hours rest and raising it didn't look or feel anywhere near as bad - although was still obviously swollen and very painful and I still couldn't weight bear. She basically said its just a sprain and although she still hadn't received my xrays, she was happy that the hospital wouldn't have missed anything, as the xrays are double checked at the end of each A&E shift. I pointed out that I had a 5th met avulsion two years ago that the same hospital missed both while I was in A&E and on the subsequent check. That time I walked on it for a fortnight before being sent to a different hospital by my GP. First hospital took 3 xrays, second took either 6 or 9 (can't remember) but found the fracture straight away and I ended up in a cast for 10 weeks. So obviously I have very little faith in the first hospital.

I have googled (I know I shouldn't, but what else is there to do when you're sat here with yer leg in the air for weeks on end) and from the location of the swelling and pain I think it could be a non-displaced mild Lisfranc injury.

Dr first refused to send me for more xrays, but after I pushed said she would put in a request and it would be up to the hospital if they would xray again after reviewing my A&E xrays. She also said my alternative would be to go to A&E at the other hospital on the basis that it's got worse, but that they probably wouldn't like it if I did that and protocol was for her to send me for xrays.

So now I have had to come home again and have to wait for a letter from the hospital to tell me whether or not I am going to be re-xrayed. In the meantime I have to just continue to struggle to get around and do the school run.

I am pretty sure there's something more than a sprain going on here, but feel like I am just going to be brushed off and left to deal with the pain on my own.

Any professionals or people who've been through simialr around who can offer some advice? I would be so grateful as since getting to the GPs and back I am now in agony again, have a school run to do in half and hour and then another this afternoon and am so scared of the pain. Sad [pathetic Moose emoticon]

Severe pain, Purple and cold foot means loss of blood supply Our blood supply keeps our feet warm and pink....
needs further review by A & E or 2nd opinion GP

P.S. dont worry if "they dont like it" at A&E, if symptoms like these continue they are under duty of care and must investigate
Im sure u would rather be painfree, with normal movement of your feet and not trapped keeping your foot elevated for weeks....

Thank you Sweetiesmum, that's what I thought. Unfortunately, since my foot looked a whole lot healthier this morning the GP said there can't be any blood supply issues. It is purple and freezing cold again now.

Hi Moosemama,
hope no further purple/cold/severe pain? My friend tripped over last week when out walking the dog and avoiding a "problem nosey neighbour". I went to see her tonight for a coffee and she couldn't even go out as too awkward and sore to walk over steps on (even with crutches). She has a two fractures in the metatarsal bones in her foot. She says it will be 6-8 weeks before she can fully weight bear on the foot. Hope your foot is a little better?

Been resting as much as possible and it's ok-ish today. Waiting to hear if the hospital is going to re-xray at the moment, with a plan to go to A&E if it gets as bad as it did the other night.

6-7 weeks is a long time to be hobbling after three children and in pain
