I sympathize - I have had the same issue for years. I'm in my mid 20s but my acne had been getting worse and worse since I was around 11! I just put it down to teenage stuff and I had my first child at 18 so wasn't surprised when it came back after that (not too bad though). Then I had my second child at 21 and after DC2 was born got cystic and painful and A LOT worse, and other symptoms started appearing or worsening.
Finally when he was 18 months I was diagnosed with PCOS from blood tests and scans. The consultant said I was lucky that I'd had my children young before the PCOS got out of control and stopped me ovulating or having periods altogether.
I think the treatment for the acne would depend on the cause and on how much you are prepared to put up with possible side effects or risks of taking the medications.
I don't think Clearasil or anything like that will help much with anything beyond a few pimples tbh.
I have had some success with Benzol Peroxide Gel which your GP should give you easily and although it says do not use for more than x amount of months I've used it for 10 years on and off with no problems. It however makes your skin very dry and very flaky and only reduces the acne a bit for me.
Roaccutane and lasers were not an option for me due to already being extremely fair haired and pale skinned, however Roaccutane has a huge success rate I think. Although side effects and risks don't seem too pleasant, as described by someone above.
If your acne is caused by hormones or sensitivity to hormones, it may be an idea to try one of the less androgenic combined contraceptive pills. Dianette is probably the best although has quite high risks compared to the other pills and some docs are reluctant to let you stay on it longer than 6 months - 2 years max. Yasmin is a good choice too, but quite expensive so some NHS places will want you to try something else first or just not prescribe it at all (my local family planning won't prescribe it at all). I had quite good success with Marvelon/Gedarel and this is quite a widely prescribed one. However obviously there are risks to taking the pill and you may not think it worth taking for this reason alone.
Lastly, when I came off Marvelon my acne returned with a vengeance! So I asked to be prescribed a drug called Spironolactone (used for the symptoms of too much male hormones in PCOS but it's actually a tablet traditionally used in heart failure and high bp - not technically licensed for use in PCOS or acne etc, but cheap to prescribe so they may be willing to let you give it a go). It has the side effect of making you need to pee A LOT , and another one you MUST TAKE WITH CONTRACEPTION as it could be teratogenic to a fetus, and it can have quite rare but potentially dodgy side effects of too much potassium in the body, although you will be monitored for this so it's unlikely to cause a serious problem. From the first few days on 100 mg a day my acne was gone, vanished. Still get the very odd spot but nowhere near what I had, and my skin is much less greasy than before, so I'm happy with it as it seems to balance all the other problems with my wayward hormones, don't know if it would be worth taking it for acne alone though.
