School failed agreed exam arrangements for autistic sixth former with misophonia

I'd really appreciate some advice please. DD has autism and severe misophonia and is currently taking A'levels. She's in the Sixth Form of the school where she took GCSEs and the access arrangements have remained the same - extra time, room on her own, use of a laptop. The school is very well aware that her misophonia is so severe that any noise can destroy her ability to concentrate and has previously agreed that invigilators will not eat, tap pens, wear heels, etc. There have been issues in mock exams with invigilators eating and the school promised they were on top of this for the actual exams.
DD came home distraught after yesterday's English Lit exam; the invigilator coughed, sniffed, mumbled and hand flapped throughout and DD couldn’t function but was unable to articulate that the arrangements weren't working. I've since spoken with the HoY and she has confirmed that the invigilator is autistic with tics and should never have been assigned to DD. The HoY has said she is "devastated" and "embarrassed".
DD knows she has performed badly and is devastated because she needs a top grade for her uni offers. We feel so let down and rhe HoY has admitted they have failed DD. Is there anything that can be done?

Thats awful OP, a real mess up on the schools part. I guess the HOY could write a letter to the Uni explaining the situation.

On another note Is you daughter getting the help she needs for coping strategies for misophonia. Uni is a noisy place. I have mild misophonia so I understand the fight or flight brain scramble your daughter would have gone through in that exam. My own kids have told ridiculous stories of bad behaviour from
exam invigilators talking, rattling and just being annoying.

For all the other exams could she hand the invigilator a pre typed note about the requirements as a reminder.

The invigilator sounds unsuitable as an invigilator for anyone, let alone someone with misophonia. Mumbling and hand flapping isn't appropriate when people are trying to do exams

She needs to get evidence from school and get in touch with the uni. Don’t leave this till a later date.

School need to put in a request for exceptional circumstances to the exam board. The school will need to be honest though. Could she wear noise cancelling headphones - bit late to get is as a reasonable adjustment now though.
