Fall alarm not going off

My MIL has had a number of falls over the past two years. About 3 months ago we finally persuaded her to have a fall alarm. She opted for a pendant alarm as she wanted to be able to wear it under her clothes.
However in the four weeks she has three falls - two quite heavy and the alarm as not gone off. Luckily other people were about and able to help.
Does anyone has experience of this and would a wrist alarm be better?

It’s worth asking if there is a local falls service she can be referred to.

Yes, unfortunately DM had a fall and the alarm didn’t work at all. She was on the floor for 2 days, as timing wise it just after DBs visit and 2 days till I went. She died after a week in hospital. We tried our very best to make the council who arranged the alarm and company who provided it aware that their alarms at best - all needed testing and at worst were not fit for purpose. They could not have cared less, which made the whole traumatic event even worse.
