At what point are memory problems dementia and how could this affect care home?

My 87 year old grandmother has been in a care home for a year. She was assessed as not needing nursing or dementia care when she went in. Recently shes had a few falls, she stopped getting dressed and the care home just leave her in bed practically naked. I go a few times a week but i just cant go everyday.

she is in hospital now after having a fall, she started vomitting blood and she has pneumonia.

i was wondering about asking them to reassess her needs. But im not sure what actually classes as dementia and whats just really bad memory.

She doesnt know where she lives - the only address she knows is one she moved out of 20 years ago. She forgets whats happened 5 minutes ago. She can talk to someone then forget they were ever there a few minutes later.

the hospital did ask her to recite the minthd if the year backwards but she misunderstood and said them forwards and missed some anyway.

if she does have dementia would this make it possible for her to move to a home with a higher level of care. She is relying on the council to fund the care so im not sure how it works!

if the care is being paid for by the council then your nan should have a social worker or social work team, you need to contact them & explain the situation. They will be able to reassess or get a hospital social worker to reassess while she is in hospital and then additional care can be arranged for her. You can also speak to ward staff in the hospital and ask for a memory test to be carried out
