Any one knowledgeable on CHC please?
I want to see if I've understood it correctly, I've read the National Framework, but the time period concerned has gone so I'm maybe not up to date with current legislation. This is a basic explanation of how I've understood it:
Patient is in hospital or rehabilitation, has complex health needs.
A checklist is completed and patient has passed the checklist so a DST assessment should be arranged within 28 days.
In the meantime a care package is sorted out, at no cost to the patient.
The Local Authority cannot lawfully ask the patient for a contribution or financial assessment during this period.
The DST takes place and the patient does not pass the full assessment so the funded care package is no longer relevant and the LA can now request the patient has financial assessment with a view to them making a contribution.
From the period of the patient leaving the hospital there should be no gap in care provided by the NHS once the checklist is completed and positive.
Does that sound right? I would really appreciate any help.
I know CHC has a very difficult threshold.
