Any advice very gratefully received as we are at the end of our tether :(

Main question is: if someone self discharges from hospital before the local authority has put a (local govt funded) care package in place, how do we arrange urgent care now they are back at home? We keep coming up against dead ends and the situation at home is pretty dire.

For context - my mum is very ill and has been in and out of hospital for months. The last pretty awful stay lasted 4 weeks. On Saturday, mum effectively self discharged (supported by my dad). This initial discharge was BEFORE a care package had been lined up by the hospital. We as a family told my parents this was a big mistake but they were determined to get her home.

She was then taken back to hospital by ambulance just 24 hours later and has now been discharged again by a&e with very strong painkillers.

My dad is unable to care for her to the extent she needs - she is not currently mobile so will need help with all personal care for several weeks at the very least.

Is there any way to get the care package put in place even though she has been discharged already?

You can pay private care companies if they have capacity to take on the calls but if your mum is that immobile they will need to have appropriate manual handling equipment in place to make sure that the care can be provided safely for you mum and the carers.

this may be walking aids, bath aids, hospital style bed, hoists and slings, raising the height of chairs and toilets etc and this type of equipment is usually assessed for and provided by community occupational therapists. Where we are they can be accessed via the council, but there are often fairly long waiting lists.

You mention your mum discharged herself the first time, but then A and E did not admit her, so presumably they must have stated she could go home with no care; is that the case?

How much savings do they have? Does she receive any benefits such as Attendance Allowance? Who is caring for her now?

If you are going via the council, contact Adult Social Care in the morning.

like a pp said, I think if she was considered fit for discharge by A&E, the hospital now aren't responsible for putting care in place.
