Employment status for personal carers

My grandparents are now in a position that they need care to enable them to continue to live in their own home. Moving to a residential home isn't something either of them want and as a family we are happy to keep them at home for as long as they are able.
We have found someone suitable to do the job and they have requested to work self employed. We were proposing to employ them, but more because we thought that was what would be expected, rather than a knowledge of carers and what they prefer to do. It will be a full time job, with us setting the hours and tasks that need doing (some personal care, cooking, cleaning, driving my gp to shops/clubs, companionship). The person hasn't formally done this job before, but has relevant experience within their own family.
I am a nanny and I have always been told nannies can not be self employed as the job description does not fit with self employment. When my mum has spoken to Hmrc today they have said a personal assistant/carer can be self employed. Can anyone tell me if this is actually correct and if so what the difference is between employing a nanny to look after your children and a carer to look after your aged grandparents please. the legal side of trying to employ someone is a nightmare! Also any tips or help as to things to include in contacts etc would be really welcome.
