Nvq level 2 help

Hiya. I'm currently doing my nvq level 2 and need to write weekly reflective logs about calls, I'm a community carer, I have no idea how to word these tho, has anyone else done these and can show me some examples. Tia

Have they talked to you about reflective practice? In nursing progress logs the student gives a brief description of what they did, then they reflect on what went well and what they would do differently next time.
There are lots of good articles about reflective practice.

I have a template I have to write what I did in the call, my feelings, positives and negatuves, analysis, conclusion and action plan. I'm just not very good at wording stuff and I'm obviously not allowed to name the service user so not sure what to call them, and if I literally just write about the whole call.

I attended client in their home. I did X and Y for them. They seemed happy to see me. I was there z number of minutes. When I left client was comfortable.

That reads like a record as you would do in work.

My understanding of reflective is more like a PP said, saying what went particularly well, and what didn't and what to try differently.

So as well as the basic log I'd think it would include things like:

Instead of asking X directly about whether they had been active which didn't elicit a good response last week, I chatted generally and they then volunteered they had been to the shops and a walk to the pond. I will try this indirect approach with other clients who aren't forthcoming to direct questions.

or

Y got very defensive when I suggested that it might help if he ate fewer biscuits next time I will try an approach to get him to the answer to improve his diet himself.

Reflective accounts need you to reflect on your performance/the outcome and what you have learned from that interaction or what you could have improved on. You would then be expected to show how you have implemented a change in your behaviour/learning on the next call and so on.
So by the end of the process you have tracked learning + skills development from the starting point to the end point.
Just stating what took place isn't enough.
Also just an FYI, don't call it an NVQ as it should be an Apprenticeship Standard with an EPA at the end 😊. It's really important you use correct terminology for these things now as you will be expected to at EPA. NVQs were phased out a few years ago.

Do you patients have a number if so you can use that as it should be covered in your work place/centre agreement and does not breech GDPR.
My nursing students would write something like:
I cleaned the surgical kit for patient 12345 who had had a spay, first I rinse the instruments in clean water to remove gross contamination and then I put them in G9 solution for 10mins. When I came to remove them I realised I had forgotten to check when the solution was made up, it was out of date. So I removed the instruments disposed of the old solution made up new solution according to manufacturers instructions and put the new date on the lid. I then put the instruments in for another 10 mins. In future I will remember to check the date before I put instruments in,but I feel please that I did notice that the solution was out of date when I came back and was able to rectify my mistake.
