Has anyones LEA broken the Admissions Code regarding school places/waiting lists?

I have started this new thread (I have one less specific under Primary Schools) because I want people to tell me their experiences specifically relating to their LEA and how their place was allocated.
Some experts on here have already been really helpful in highlighting the Admissions Code (the 2010 is still current for this year) 1.50 in which it states:

"If a parent has not responded to the offer of a place within a resonable time the authority must remind the parent of the need to respond within 7 days and point out the the place may be withdrwn"

My LEA (West Sussex) and some others do not do this, if someone does not respond to the offer they assume that this place is taken up and this childs name goes on to the school, often the school have no idea that this place has been 'positvely accepted'
Another local LEA East Sussex do all in their power to follow up on these families especially when it involves a waiting list, these places are then withdrawn and then allocated to the child on the waiting list.

This means that any LEA who does not follow this guideline is in breach of the code and is potentially holding many places up by people who have not responded. Even those involving a waiting list which means it is not following it's own Parental Preference guidlines.

Who else has an LEA that does not require an acceptance, is anyone on a waiting list that the school suspects they may have parents who have not responded to their letters?
