Medical mistake- advice please

Hi. I'm not an HCP but need advice from.one so hope it's ok to post here.

A family member has experienced a mistake in hospital where the wrong body part was amputated. It wasn't a limb or organ so won't be life threatening/altering but it was a healthy body part amputated in error. The surgeon apologised to my relative and the hospital said it would be investigated as a "never" event.

The hospital.launched an investigation and I gather that this has been conducted by a consultant and nurse from a different department. The report is now completed apparently and will go to the CCG.

My relative is concerned that no one has asked him for his account of what happened and how it has impacted on him.

So, if you work in a hospital, is this normal.practice for this sort of error? My relative doesn't want heads to roll but does want an indication that the hospital are taking it seriously and is concerned that the investigation is being done by 2 internal HCP s and that he hasnt been involved or spoken to. Any advice gratefully received

Never events would usually go through clinical governance. The clinical governance lead would deal with the patient face to face and take and account of what happened, keep them informed etc. It's an important part of the process.

Did your relative go through PALS?
