Decree Absolute copy

I have been divorced for over 20 years and i lost my Decree Absolute and trying to get a copy, I can't remember the court or case number and i am filling in the D440 form, I have a few questions regarded filling in the form , first, " Full name of the applicant (the person who issued the proceedings " this is me should i use the last name which was my married last name when i was getting Divorce ? its not something i use now i have gone back to my birth surname name.

Questions 4 to 10 I can't remember the court or case no so i will leave them blank , The others are dates - Date of separation- Date or year the petition was filed- Date or year the decree nisi was pronounced - Date or year the decree nisi was made absolute- Well not really sure about the dates for all of these , I have an idea of the year of the Divorce that's it .

I would think that the name if applicant would be the name you had when you petitioned for Divorce so that they can search for that name in their records. There's a section further down where you can put your current name and address.

They will search in 10 year blocks so if for instance you put down 1990, they will search from 1985 to 1995.
