Moved from Metformin to Dapaflogozin..... blood sugars all over the place!

I have been type 2 for about 10 years, and in the last 18 months have massively reduced my HbA1C by a low carb diet and I was on 1 x 500mg metformin XR a day. I had a letter from my GP surgery last month and was offered the chance to try different medication.... so I had my bloods done and spoke to a clinicial pharmacist (think it was someone at Astra Zeneca) rather than my usual diabetic nurse. My HbA1C was 44 so they suggested moving from the Metformin to 10mg dapaflogozin which I've done in the last few days. 1st day my blood sugars were usual (between 5 and 7) but yesterday and today they've been much higher than usual for me (between 7 to 12 each reading) and I've had the most horrific headache. I was told to go back for bloods in 12 weeks but I'm really reluctant to carry on if my HbA1C is going to be a lot higher before I see someone. Has anyone else done this change and how long did it take for your blood sugars to stabilise?

44 is very good control! Not sure why they have suggested that change. You could even have stopped all meds I would have thought. Are you overweight?
could you be unwell and that be putting your sugars up?

Have they never heard of the phrase “if it ain’t broke, don’t fix it”??

Did they say why they would do this? Is it part of a piece of research possibly (that should have been explained to you if it’s the case)? I don’t understand why they stopped your metformin, as it’s known to have a beneficial effect beyond glucose control (reduces the risk of certain cancers, provides cardio protection, gives some protection against dementia), I would speak to your usual diabetic nurse.

Also, really well done on the dietary changes you’ve made, as they must have really helped too.

Low carb diet is advised against on dapaglifozin due to the DKA risk. Are you drinking a lot as the headache could be dehydration? Just switch back if you were happy on metformin but if you have any DKA symptoms seek urgent medical advice.

Thanks for the advice. Having read the patient info leaflet, it can interact with the blood pressure meds I'm on so I've booked a phone call with the surgery pharmacist to check it's suitable. I feel quite annoyed that I've been dragged into research without realising it. Was very happy pottering along just as I was.

I hated metformin
Tried dapoflogzin.. hated that too.
Then Alogliptin .. nope didn't like it.
Got Mounjaro and completely reversed my type 2 in 3 months.. added bonus lost 4 stone.
Win win.

I know this is an old post but I could have done the OP myself. Got the call yesterday any one else had same issue and how have you found it?

Wakemeupbe4yougogo · 18/08/2025 19:00

I'm still not convinced it's been a success but I'm due to have my 6 monthly bloods done this month so will find out if it's been working or not. My blood sugars are reading between 8 and 10 most mornings but generally under 8 for the rest of the day - so I'm fairly convinced that my HbA1C will have crept up.
