Type 1 and general anaesthetic

DS is type 1 (omnipod and dexcom g6) and is having surgery in a few weeks. I've read the NICE guidelines and have a phonecall planned with his DSN beforehand. Would love to hear any advice anyone else has.

It's a short op, and no overnight stay needed. Any tips on what bg to aim for that morning, do we expect highs or lows after?

You’ll be given all the instructions for fasting and blood glucose management when you attend the anaesthetic preassessment clinic, OP.

Can't speak for the adult side of things but for my child she kept pump on while in surgery- it was very short, and we self managed while in there. If he's not happy about anything they insist on doing, ask that the nurse calls the diabetic consultant on call. Your DH will know more than any nurse on the ward about his needs.

Do you have a pump specialist nurse? They should advise.

For me they recommend:

"70% is absolutely fine for fasting, in terms of use during the operation that comes down to the anaesthetist who will discuss it beforehand, if you are able to keep pump on during the operation then suggest reduce it to run slightly lower at a temporary basal of 60%. "

He should be put first on the list as a diabetic, they did with me, check at pre-op.

I've just had eye surgery inder general anaesthetic as a day patient (last week). I'm on injections, not a pump, so maybe the advice will differ a bit, but I was told to continue with my basal (long term) insulin as normal and aim to keep my glucose level. Yes, ok then... I was planning to run a bit high - about 8-ish if I could, to give me a bit of leeway. As it happens, it all went fine until I was being prepped for the op (I was first on the list because of being T1) when it suddenly started to plummet and dropped from 9.2 to 4.1 within about twenty minutes. I've been having some horrible ninja hypos lately, so I spoke to them and they put me on a glucose drip which sorted that out.

Afterwards it was pretty much as you'd expect - bit sick, very tired, and my readings were all over the place for a couple of days, but generally higher. I think that might be due in part to the steroid eye drops I'm taking, but I'm just keeping an eye on it and doing correction doses if I need to.

Good luck, hope it all goes well for you.
