What could this be? Dementia?

We are worried about my MIL. 4 years ago she became really absent in conversations, a stutter appeared and she did a few erratic out of character things. She seemed no longer able to cope with simple things like a phone call or Skype. Since then her mobility has declined, she shuffle walks and motor skills are impaired- for example she now can't do up her jacket or struggles to make a cup of tea. Her facial expression is vacant and she stares a lot. Often she has long periods where she stares into space or down at her hands. She gets involved with conversations but rarely initiates them and her answers are bland and often copying someone else's answer, parrot-like. She has had several falls over the past 2 years and admits her balance is bad. This last visit we have noticed that she has an odour and she says she doesn't wash often as it dries her skin. She is 73. She has seen a neurologist and a doctor but nothing seems to have come of it. They have told her not to worry and nothing significant is wrong but to us her whole personality has changed. We can't seem to talk to her directly about this as she is very closed and defensive. When she has opened up after much prodding, she says she is living with significant stress (,her partner is an alcoholic) and that is the cause of her symptoms. We do not have a POA and to some extent my DH is in denial too, he believes it is primarily a mental health issue and then a motor issue . Could this be the start if dementia? What's the best way to get a diagnosis? She is getting a brain scan soon but I have no idea when and what that would tell us and I'm not sure she would share the results with us. Also worth mentioning that we live in different countries so we are not able to do much on a day to day basis. Thank you.

Yes, but also Parkinson’s with the loss of facial expression.
If she’s going for a brain scan then surely you can’t say nothing has come from the GP visit `?
If you are concerned then I’d contact social services and ask for an assessment.

Thanks GoodnightGrandma. She has told us that the neurologist said it was unlikely the scan would show anything at all so that's why I feel we are back at square one - but we will just wait and see. They can't see it as urgent however as she was referred for one over 3 months ago ( not NHS healthcare).

I’d her SS involved then. Sounds like she needs as assessment.

This sounds a bit like my mum, especailly the vacant expression and repeating things back, also loss of mobilty//balance and falls. I've got a couple of threads going on in the Elderly Parents forum topic, as she's been in hospital/care home 'convalescence' (4 week assessment) since end of November after a fall in which she broke her arm.
I've got a Parkinsons suspicion, but no medical people have suggested it.
About 3 years ago she had an assessment at the memory clinic (I can't remember what instigated it, probably low mood), and they didn't think it was dementia, more like depression. 2 years ago she had a CT scan of her brain, and they didn't seem concerned. She'd done OK on the memory tests too. They tried her on various anti-depressants, but only for short periods of time.
She had a recent CT scan after her fall, and they didn't think there'd been much change over the 2 years. They are going to repeat the memory tests soon.
I think lockdown/isolation has had an impact, but who knows, it may have happened anyway.
I'm so upset about it, as no-one seems to be interested in a diagnosis, and I suppose it wouldn't make any difference to know, as there's probably no treatment for whatever is wrong. I don't know where my mum has 'gone' and I'm mourning for our past (very good) relationship.
I hope you get some answers, and if I do, I'll come back to this thread to share.

Thank you @EyesWithAFace and I'm sorry for your situation too. It's incredibly sad because, like you, I feel we have been losing my MIL for years and she has become a shell of the person that she was. And yes, it's so hard to know what is depression, what is old age, what is a condition. I find it very frustrating that it can't be escalated and a diagnosis searched for but I get the impression now that unless something dramatic happens such as a bad fall, the health system where she is will just let her carry on with no support. I certainly will come back and update you with the results of the scan and any suggested treatment plan. Sometimes I feel that I am in an alternative universe and screaming 'Look something is really wrong here' but everyone we talk to (including herself) dismisses our concerns. Frustrating. Take care x
