“Shouldn’t we do a large, prospective, double-blind, placebo-controlled study to “prove” it works before adopting yet another treatment that will not work?”

There are several reasons why such a study would likely be unethical to conduct at the current time. We agree that further studies can and should be done but placebo controlled RCT’s should be avoided due to the following:

Currently, a total of over 3,000 patients have been included within numerous randomized, controlled trials with the overall signal of benefit in important clinical outcomes strongly positive with tight confidence intervals. This would make the likelihood of causing significant harm to study subjects in a medical research trial using placebo to be unacceptably high given excessive morbidity and mortality associated with COVID-19.

Further, the WHO ACT Accelerator Program (www.who.int/initiatives/act- accelerator/about) sub-section focused on treatments for COVID-19 and headed by UNITAID has hired research consultants to identify and perform a global systematic review and meta-analysis of all active ivermectin trials in COVID-19. The consultant anticipates having results available from several additional, large clinical trials within the next 4 weeks, and predicts the accumula- tion of sufficient patient data in these trials to reach a conclusion and recommendation for or against use of ivermectin in COVID-19 during the month of January 2021. Preliminary analyses by the consultants were recently presented at an international research conference and all the available trial results at the time strongly supported the efficacy of ivermectin in COVID-19. If, based on the projected amount of trial data in the coming month, a recommendation for use of ivermectin in COVID-19 is issued by the WHO, any planned subsequent placebo-controlled trials would have to be terminated.

This is a bizarre development since Ivermectin is widely used in Africa for other reasons as approved by the WHO:

I've been reading about ivermectin. Some countries are already trialling it and so far it looks like a very plausible treatment/prophylactic.

Lots of potential which is not being realised. I suspect big pharma might have something to do with that.

It does have some unpleasant side effects though too - when I had scabies several years ago I googled treatments as the lotion gave me a horrific allergic reaction (my eyes swelled up etc) and it's not used in the UK as far as I remember for that reason.

Interesting Ivermectin is the active ingredient in the cream I use for my rosacea, they are not sure why it works but think it may have an anti inflammatory action (as well as killing idemodex mites). Rosacea is a disease caused by the sufferers immune system, just like the severe symtoms of Covid.

This is an interesting run-through of a selection of Ivermectin studies world-wide by Christy Risinger, MD (Texas).

Brief, well-presented and easy to understand - all good so far Smile

General theme of Dr Risinger's balanced comments on each study: "These conclusions look promising but here are the shortcomings of this study - more research needed."

Her previous video covering the same ground was swiftly deleted by YouTube so I hope they leave this one up.

(In case anyone is unaware, there is heavy censorship by YouTube, Twitter and Facebook of any content that might be perceived to support treatments other than those authorised by the WHO and the US CDC. Now that WHO ACT-Activator has registered a review of evidence about Ivermectin, maybe this video will escape the YouTube scythe.

This is a rather hypocritical stance by Silicon Valley Social Media, given that Fauci has now twice admitted that he lied to the US public, first about masks (bad then good) and now about the percentage of the population he estimates need to be vaccinated to achieve herd immunity - up from 60% to over 85%.)

Hhmm, one reason against using it, especially as a prophylaxis would be the fact that its very slow to break down and would accumulate in the aquatic environment, theres damage being done to aquatic life at the moment from flea and tick treatments on pets, if millions of people took ivermectin it could be a big problem, just a thought.

The "Ivermectin meta-analysis by Dr. Andrew Hill" is crap. It's several levels of almighty crap.

Just a few things to point out... it claims to do GRADE assessment: where is it? That means no assessment of quality of the trials.

You don't combine different study designs (RCTs & observational study designs) in a meta-analysis forest plot. Norty. Crap.

I only checked the 2 largest of the 4 RCTs used in the forest plots of the slide summary (mortality outcome). Elgazzar & Mahmud. They are both preprint/unpublished, not been thru peer review, and not actually that big (n=about 600 patients). Elgazzar's arms are

ivermectin + cocktail of other things
vs.
hydroxychloroquine + cocktail of other things

There was no control group at all, and yet we know HxyChl leads to worse outcomes for covid patients.

Meanwhile, Mahmud's arms are

Ivermectin and Doxycycline (an antibiotic)
vs.
standard care

You can't put Mahmud & Elgazzar in the same pooled meta-analysis, bogus scientifically, like making apples & pear salad to decide how sweet oranges taste.

I haven't found the other 7 RCTs supposed to be part of the meta-analysis, I can't be bothered to look harder. Probably tiny & crap. Oh, and there's no IQR ranges or confidence intervals on HIll's column charts.

More reasons why using ivermectin may be very bad.

It is really useful to have comments that point out flaws in the research for lay people like myself, so I do appreciate your taking the time to look at that, MedSchoolRat.

The Swiss Policy Research article mentions that three more studies will be added to Andrew Hill’s meta-analysis and that final conclusions will be published this month.

I forgot to post a link earlier to developments in Belize, which decided to make Ivermectin available under medical supervision about the same time that South Africa banned use of Ivermectin.

These countries seem to have adopted opposing policies in response to the same situation, ie. that their populations were already using Ivermectin as a possibly effective treatment and/or preventative for Covid-19 in the absence of official Government guidance. Hard to know to what extent these decisions were made on clinical vs political grounds. Both countries must surely have access to the same data?

The Belize response at least provides an opportunity for more data to be gathered.

This link is to TrialSite News. It provides more information than any other articles I could find but there is also a heap of evangelical advocacy for Ivermectin to filter out. If I could have found an equally informative but less biased source I would have been happier - just be prepared to grit your teeth and skim over all the “wonder drug” type references.
