Staff member's spouse has been told to isolate by test and trace. Standard procedure, staff member carries on as usual, unless spouse develops symptoms.
Spouse does now have symptoms, so rest of household isolates for 14 days unless/until spouse gets a negative test result.
Spouse is refusing to be tested, as they have to isolate anyway so don't see the point.
Staff member has to continue to isolate even though if the spouse was tested ("symptoms" appear to be a mild cough and runny nose), they could be back at work.
I don't think there's anything for the employer to do except wait it out? I'm sure the fact that both spouse and staff member have jobs where isolation is on full pay is irrelevant
