correct timing for day '21' progesterone test (after pregnancy loss)

My gynocologist has referred me for progesterone tests after a mc at 6 weeks. I have some other symptoms of low progesterone (like spotting before period).

Prior to the mc my cycle was usually 25-26 days, with ovulation (according to opks and fertility friend app) on day 12-13, so he specified to wait a cycle then have the test on days 18 and 21.

Last cycle (the first post mc) I ovulated later on day 16 and my cycle was 28 days long, this cycle It's day 14 and still no positive opk. I'm therefore worried days 18 and 21 will be too early to reveal a problem if there is one?

I'm not sure if the point of testing on 2 days rather than only on day 21 is because my cycle isn't always the same length, or because he wants to compare the levels? How should I be calculating the test days for the most useful results?

Any advice from anyone who understands how these tests work would be gratefully received.

Ideally you'll need to track ovulation with temperature and OPK to nail down a correct day for 21 bloods. I'm not sure why they have sent you for this after a miscarriage though. You are clearly ovulating. How long have you been TTC? Is this your first?

TTCing for first child, aged 35 (almost 36). I conceived after TTCing for 7 months (I also had an early mc at 5weeks 18 months ago when not actively ttc).

The test is being carried out as one of list of post-mc investigative tests, fertility tests will be done if needed after that. I think the suggestion was that the progesterone levels could be high enough for me to ovulate, but too low to sustain pregnancy/implantation?

I haven't tracked my temperature this month as I've had the flu and had a temperature for a week. What is day 21 based on - counting back from the start of your period, or days past ovulation?

It should be done 7 days AFTER you ovulate. Obviously on a regular 28 day cycle with textbook ovulation on day 14. That's day 21.

Still though. You were able to get pregnant. I do not know why they are testing your progesterone levels. Unfortunately miscarriages happen as a natural process sometimes and if you're in the UK recurrent testing is usually not offered until you've had 3 losses.

I've had to fight for these tests after 18 months of TTC, waiting months and months for a referral to a fertility clinic and whilst my progesterone level was very low I managed to carry a pregnancy to 10 weeks last February. Having a mmc is horrific but unless you follow up your tests with actual help then it's pointless. Usually referral is after 1 year or 6 months if you're over 36. Good luck.
