Hi girls,

Just to give you some general background before I ask the question.....I'm off the Pill since Nov 05. I already have a DS who is nearly 3. My cycles after coming off the Pill have been really awful, ranging from 60 - 125 days, and my gynae thinks I probably didn't ovulate on very many of them. He has ruled out PCOS, my hormone levels are all quite normal, I've had an internal scan to rule out anything else. Basically everything else is clear, just that I'm not ovulating regularly.
So he put me on 50mg Clomid, to be taken days 4-8 of my cycle.
It has worked well, my cycles are now pretty regular and I am ovulating as confirmed by blood tests. (still not pregnant though! ) I am not being monitored on the Clomid, just going to my GP to get 7dpo blood tests done.
My question is this: on my last cycle, I got a progesterone level of 104 7dpo, which is obviously SUPER, my gynae had told me that anything over 15 confirms ovulation, and he would like me to be over 30 on a medicated cycle.
Yet, I started spotting at 10dpo with AF arriving 11dpo.
My GP is mystified, she cannot explain to me how or why I could have such a high progesterone level for starters, she says the only time she has seen levels that high is in people who are pregnant, and she also cannot explain the very sudden drop in progesterone, why I could start spotting 3 days after getting a level that high, as spotting normally indicates a lack of progesterone.
She has referred me back to my gynae, but I can't get an appointment with him until May.

So I decided to come on here and ask you all whether you can shed any light on the matter, as I've often found that people who are going through the same thing are much more knowledgeable than the medical professionals!
Any ideas????

What are your hormone levels with regards to your LH and FSH?. Have these ever been compared against one another?. If they were not I'd ask some awkward questions.

I would actually seek a second opinion from another gynae.

Have thyroid problems been ruled out?

Spotting between periods ought to be investigated as well to ascertain the cause of this.

Another thing as well; you ought to be monitored closely whilst on the clomid as it is powerful stuff. Its not always suitable for all women with ovulatory problems.

If you're not being monitored then there's no way of knowing whether an egg is actually leaving the ovary or not (hence the need for internal ultrasounds whislt on clomid).

All this anyway is beyond a GPs general remit. You need to be seen far more often as well, I would seek a referral to another unit where you can be properly monitored.

Hello,

My hormone levels are okay, both at cd3 and 7dpo. I haven't got a copy of my cd3s here in front of me, but my 7dpo were as follows:
FSH: 3.6, LH: 2.9

Thyroid problems were ruled out early on by my GP.

I'm actually going to my GP again on Monday for 7dpo bloods for this cycle, and I'm going to ask to be referred to a specialist Fertility Clinic rather than back to my gynae.

Hi posh,

What you ideally need to have redone is your Day 3 tests. Not really surprised to see that your LH and FSH were low late in the cycle; these two are higher if measured earlier and therefore best tested earlier in a cycle.

Think you are wise to ask for a referral to a specialist fertility unit.

I'm home from work, so I've located my last set of day 3 tests:
LH: 6.0
FSH: 8.0
Oestradiol: 106
Progesterone: 1.8
Prolactin: 395

Any comment on whether these are good or bad? My GP told me they were fine, but at this stage I don't know who to believe!

I would like a specialist to look at those results for himself but your prolactin level looks very high.

The month before that, my prolactin was 451!!!!!!!!!!!!!!!!!!!
My doctor did express concern about it then, but I am under a huge amount of unavoidable stress at work, so she thought that it may be related to that.

When I met the gynaecologist, he said that we wouldn't find the answer to my problems in the hormone levels and that the bottom line is that I have an ovulation problem, therefore Clomid will solve it. And this guy is a Professor of Gynaecology and Obstetrics, and is the Head of the local Maternity Unit. That's why I got an appointment with him, because he is the most highly qualified guy in the area.

Thanks again Attila for your help, you've been very good to answer all my messages. Like I said previously, I have 7dpo bloods tomorrow and I'm going to get a referral to a specialist fertility centre. Maybe they could shed some light on things!!!!!!
