I don't know a huge amount about TTC, but this is our situation:

One child already, conceived naturally.
I am in my late 30s. We have been TTC for about a year, mostly getting the timing right, so I began to take tests as follows:

Positive CD21 test.
Clear swabs.
Clear pelvic ultrasound.
Have had a course of tablets for Vitamin D deficiency.

Am fairly confident that I am ovulating.

What would the next steps be? Would it be medication, ICSI, something else or possibly IVF? We can pay for treatment, so am keen to proceed towards this in early 2014.

We were in a similar position (minus the vit D issue). We were referred straight for IVF by the NHS, although we paid for some private rounds of superovulation (drugs) and IUI before we got there as I just didn't feel ready.

My first IVF round did get me pregnant, although I miscarried, so clearly there is some undiagnosed problem that is overcome by the IVF process.

did you already have a child?

Sorry, no. I meant similar position as in the test results didn't throw up anything.

ok, thanks.

Do you mean Clomid etc when you talk about superovulation drugs?

Is there anything they can do on the male side?

I actually took something called Letrozole, which is supposed to have a similar effect to Clomid. It didn't do much for me - just made me ovulate earlier or later than usual. In hindsight I don't think it was worth bothering with. If it had managed to get me to pop out 2 or 3 eggs, all it would do is give the sperm more targets.

Has you partner been tested? What were his results like?

For male factor issues, the usual advice is cut down alcohol, smoking and eat healthily and keep the balls cool (loose pants, no laptops on laps, etc). Vits seem to have an effect on count too.
