Is Clomid the right / only option?

Hi, I wondered whether anyone has been in the same situation as me and could offer any advice? We decided to TTC #2 almost a year ago having had no problem at all with #1 - we were so lucky and it happened without even needing to try.
Since trying again, my cycle has changed and although I still have a very regular cycle, always 28 days, I get spotting throughout the 2ww. It usually starts 4 days before AF is due and I get horrible PMT - poor OH, I must be a complete nightmare. This month, it has started 4DPO and I am very crampy and horrible to be around.
I have had bloods done which show good LH and FSH levels but low progesterone levels although I do seem to be ovulating each month. The consultant has indicated that the only real option for me is to start Clomid. I always thought that helped if you weren't ovulating which doesn't seem to be my problem?!
I'll obviously try Clomid if it really is the only option but am just a bit nervous about the increase in potential to have twins! Would love a big family but just couldn't afford it.
I have tried some dietary changes and taking a B6 supplement but it hasn't made a jot of difference.
Has anyone been in a similar situation? Any advice / thoughts gratefully received.

You need a diagnosis first and foremost.

When did you have your LH and FSH levels tested and compared against each other?. This test should ideally be done on Day 2. Is LH higher than FSH and have these tests been repeated?.

Have they made any comment on the spotting; something is causing this to arise and it should always be investigated (its often not serious but it should be checked out all the same). Do they think the spotting is linked in any way to the low progesterone levels or the LH and FSH levels?. I would ask if they are linked. It may be that you are not ovulating when you think you are and you may well not be 4dpo at all; it is also quite possible to have periods without ovulating. Ovulation is not an exact science and a woman can ovulate earlier, later or not at all in any given cycle.

Clomid's main job is to make the ovaries work harder. It is usually only given when there is definitive proof that ovulation is not happening regularly.

Your DH also needs to be tested in tandem with your tests being carried out; male factor problems need to be confirmed or ruled out.

My LH & FSH levels were tested on day 2 and showed good ovarian reserves and were the right relationship - the gynae was happy that there was nothing there to suggest a problem.

I am having my progesterone levels checked again this month but at 3mg last time they were checked at day 21, they are on the low side.

I also had an u/s done recently which didn't flag anything except a big fat follicle. My last smear was also ok, done just a few months ago.

The gynae has suggested that as my cycles are so regular, and the fact that my lh/fsh levels are fine that I am ovulating. I do always get a positive surge on the opks and lots of the fertile stuff and cramps.

He thinks the problem is that not enough progesterone is being produced in the 2nd half of the cycle which is causing the spotting before my period is due. Not sure what has happened this month though, seems to be different again!

I wondered whether protesterone supplements would be effective but this hasn't been suggested as an option, just the clomid which I find really confusing.

I'm due back to see the gynae next week and just wanted to go with as much information as possible so I'm asking all the right questions.

OH hasn't been checked out yet but we'll go down that route once I have some answers.
