Afternoon ladies.

I wonder if anyone has any advice. After 18 months actively TTC, last month I suddenly had a bit of a meltdown when I realised I was 37 next birthday and still wasn't pregnant and that there was likely to be an Issue. So off I trotted down to the GP - who I haven't had a whole heap to do with before as I am luckly to be in robust good health, touch wood. She advised me to come in on day 16 of my cycle for a smear (which was due), and "you might as well have some blood taken then as well". I asked what it would be tested for and she said general stuff - liver and kidney function, thyroid, diabetes, rubella immunity and anaemia. I asked about hormones and she said they would also test for progesterone.

Now I'm not an enormous expert, but I ventured that testing me a mere 2 days after ovulation would produce a low result and she said they would test it anyway. So the results came back, including the smear, and they were all normal, apart from, the receptionist said, the progesterone which was "very low for a day 21 test" (it was 3.4). I told her it wasn't a day 21 test because it had been taken two days after ovulation, and I asked about day 3 FSH testing. She said I would have to come back to chat with the doc (sorry this is long), and that she had never heard of such a test. Fair enough - she's not a doctor.

So I've just been back and the first thing she said was "How long before your period was the blood test?" Clearly she concerned about the low reading so I said I wasn't worried because it was only 2 days after ovulation, when I knew it should be taken 7 days after. She was adamant that no, it should be taken 7 days before your period starts. She also gave me some results for FSH and LH, again both of which sound ok (49 and 7.6) except of course they were taken on day 17 as well (day 16 was a Sunday so I had them the next day). We talked ourselves round in circles with me asking how did she know if it was a good FSH result if it wasn't taken on day 3, or thereabouts?

The upshot is that she has ordered me back for another progesterone test "seven days before your period" (which of course will be a bit of guesswork) and she discounts the day 3 thing altogether. Nor has the word "ovulation" passed her lips. I'm confident I am ovulating, but I want to know if I have aged eggs!!!

And MrA is going for a jizz test soon, bless him. He has to drive 30 miles with a pot of spermz to hand in at the hospital.

That's by the by. Does this GP sound as clueless as I think she is, or is it me? I have asked to be referred anyway, which she's said she'll do as soon as this blood test is done and the SA result is in, because the fact remains I am still not bloody pregnant not matter how much blood is syringed out of my arm!

Apologies for the length. Any thoughts?

Hey rie. Sorry to hear you've walked away from your GP feeling bad :( I know it must've taken a lot for you to go.
Ime GPs are rather clueless about fertility, hence why it's so important to get the specialist referral.
You're right about the 'day 21' bloods, as others have said it's ideally 7 days post ov/7 days pre-af but who has the classic 28 day cycle with a 14 day luteal phase?! The fsh/lh can effectively be done at any point in your cycle, they look at where they should be within that phase and how they are in relation to each others.
For us, my progesterone wasn't convincing - around 15 the first time! Do the infertility docs sent me off with blood forms for 7 days before my shortest cycle and 7 days before my longest cycle to try to 'catch it'. Managed to get it in the end, 28 :) so if they're not happy they will repeat your bloods any ways and refer you for other investigations like fanjo cam and Hsg. Hope it goes ok.

Yes - would agree that the 30-mile drive is likely to make SA a bit pointless.

Try not to pre-empt the tests, Ariel. And fertility is a lottery - even vegetarians could suffer from a low sperm count - in fact vegetarianism may be an issue. I have read that a lot of soya or dairy can have a negative impact on eggs and wrigglers, and my Chinese acupuncturist often reminds me to feed my DP red meat...

But - the fact he doesn't drink or smoke is really good.

Also - you may have something simple, like a blocked tube or low progesterone levels which are quite easy to treat...

Or nothing, which might be very tough, but at least you will have found out.

Sorry - feel a bit unhelpful. TTC is focker, whichever way you fry the bacon (or soya substitute).

I will pop some steak in his mouth as he sleeps. Doesn't eat a whole heap of soya, to be honest, mainly veg and derivatives . And some fish (locally caught, line fished, natch)

Tis a focker alright.

How about if he drove really really fast?

Give him lots of fish then - and nuts and seeds (in muesli?), plus some manvits if he'll have them. I assume you're doing the usual vits?

If he drove fast and it was under an hour then you'll be fine! (Or did a sneaky jizz in the carpark... Wink)

I wish you both the v best. No one deserves this crap experience. I'm pretty much at the stage where I'm going to jack it in but I've had one child (and am no spring chick) so I'm very fortunate.

You will get there - I really think you will (sometimes you just need to hear someone say that - and the tests are a good step forward. I certainly feel better knowing that SA was a slight prob but there's nothing too serious going on health-wise and perhaps just some good luck is needed!

Not ideal but would he contemplate doing it in the hospital toilet?
We were told it had to be tested within an hour and kept at body temp until you dropped it off. I had the tub clamped between my thighs while we drove fast to hospital Grin

My partner did it in the hospital loos (non-driver/shitty public transport/hospital miles away). I don't think it's that much of an unusual thing to do, actually. Would that put him off too much?

I think it would yes.

We can get there within the hour. We got there in 40 minutes the other day and that was to the city centre, not the hospital, which is on the outskirts this side. The PCT knows that people will be doing this distance to deliver the JIAP - I think if it was a problem, they would be advising chaps specifically to do it in the "tossing off room" as a friend once charmingly called it.

Hi Ariel.

I'm the same age and recently went to my GP about not have a BFP after 9 cycles.
I had to push it a bit but DH had two SAs done about 6 weeks apart. After the results for them the decided to get me tested. As I have a 35ish day cycle I had a 'day21' test on day 28 and a day 2 test. I had both of these done at the hospital which is on my way home from work. I also had vaginal swabs for infections and a blood test for general immunity.

GP was happy with the results but has referred us both anyway.

My tests came back fine. DH has plenty of chaps, just not heading the right way.

With regards to being a vegi, as we both have been vegi for about 15 years I did some reading up. One problem can be the low levels of zinc in a vegi diet. So I have got the DH to take a multivitamin with zinc. Pumpkin seeds and brazil nuts are a good source of zinc.

Thank you Pipbin. I have just bought MrA some selenium and zinc supplements. I can scatter them over the steak!

All this is assuming his SA is a problem of course. It could well be fine.

So I got the referral letter already. I had a form which advised me to have sex and avoid crack cocaine etc, and a long form to fill in.

It seems the GP pulled her finger out after all, bless her.

Well your job involves high levels of physical activity, doesn't it? I would say you were moderate-high for exercise (based purely on occasional things you've said).

How much do you actually do?! moderate three times a week for at least 30 mins is the recommendation....

Well I don't go to a gym or jog for fun or play hockey or anything like that. However I walk everywhere and you know what I do for a living, which is quite strenuous, so I am quite fit I think.

Well with the activity from work and the walking, just put down how much that is in total, like how far you walk every day. It's just them checking you don't live like a sloth! No need to worry on that score :)

Yes, just because they've called it exercise, they don't really mean occasions on which you are wearing a netball skirt. You live a highly active life, which is sensible info to give them.
