Fecal impaction, so much trouble and pain to start feel desperate

Hello, has someone of you have so much trouble like me? On February my daughter started to complain she has a difficulty to push out her poo, sometimes soiled pants, but not always, her tommy was a bit painful, I did not think of anything bad but kept an eye on her, at that time I would not think what is going to follow, complete nightmare... At the beginning of March I understood something is wrong, and we made appointment at the GP. I started to keep my diary on everything she eats/does or does not. GP prescribed Lactulose 15 ml twice daily, she took 3 big bottles of Lactulose and no improvement and her tommy started to pain more often and it worsened, next GP precribed Movicol, and told to give 2 sachets daily, I asked to do some screening as I felt (my intuition) how can the tommy be so sore after using so much medicine, doctors squeezed her tommy and confimed it's constipation but I felt it's something more... 2 sachets of Movicol did not help, the pain remain the same and her bowel did not move for 4 days, we went to GP every second day total 5 times and 3 times at emergency at the hospital, all doctors advised Movicol of 2 sachets and one doctor prescribed Dulcolax for night 5 ml (first night worked, after that no change) but her pain started to worsen and then I felt it's not a constipation but it was already fecal impaction... At the last time at GP doctor said we did everything what we could, I felt like in hell, we missed school for 3 weeks, I started to have serious issue at my work as I have to explain my employer in detail every week, what is going on... and nobody could help me. Then I started to give a dose of Movicol following its instruction for fecal impaction starting from 4 sachets up to 12 sachets, then on the second day of 12 sachets diarhoea started and I started to reduce, as result no bowel movement again, I took her to emergency as her condition did not improve at all, and they finally took an X-Ray and confirmed she is impacted! I made some Micralax enemas, all stools were always soft and light as she is on balanced diet, 1,5 litre water, herbal teas, smoothies, fruits, for some months we both eat porridge (without milk) in the morning - oats, linseed abd bran mixed with hot water, plus Linwood milled seed mix - flaxseed, sunflower, sesame, pumpkin seed and goji berry mix on top of it with low fat bioyogurt, this we eat for several months already. I started to hate GP doctors...At the emergency they told to give her Movicol the same plan ( 4 to 12 sachets 1 week plan) I started to do, at the first week of the plan result was diaarhoea in really large amount, then started to reduce, but when I started to reduce, no bowel movement, then doctors told to do second week the same plan again up to 12 sachets, this time on the second day of 12 sachets she started to scream of pain, and I called ambulance, she was taken to emergency where they told she has crumping from Movicol and precribed Buscopan for spasm relief caused by Movicol. We also had appointment at Peads and we were advised to adapt Movicol. We started the third week plan again up to 12 sachets but now she did not go to toilet for 7 days and we straightly went to emergency and Paeds again, at that day we were put into the hospital in peadiatrics we stayed there for 3 nights from last Tuesday till Friday she was given 12 sachets of Movicol there and also 2 stronger enemas (result all her stool was diaarhoea like and soft light), and was sitting on toilet to train her bowel, On this Friday after 2 week no-toilet period she strained 1 very tiny poo for two times, but nothing normal. On Friday we went out of hospital with instructions to have 12 sachets of Movicol and 10 ml of Dulcolax in the morning till this Wednesday when we have a check appointment at the hospital again. Now she takes 12 sachets of Movicol from last Wednesday and 10 ml of Dulcolax in the mornings, and nothing! She sits on toilet several times a day, a week ago her bottom passage also started to sore and she started to be afraid so much, I have talked a lot to her while she realizes that she needs to overcome the fear, and she says she does, but nothing comes out... Yesterday I put Glycerol suppository, just a tiny amount came out, nothing normal again.
Thanks for reading, I would be thankful for any thoughts from you... Sorry it's a total mess as it's a long long story...

I don't know if this will help at all but your list of foods seems quite heavy on fibre but light on fat with the exception of the seeds you mention. If you're treating an animal for blocked bowels, for example, oil is a traditional treatment. Literally 'greases the wheels'. So - if you're not doing so already - you'd put cream on the porridge, plenty of oil in cooking & lots of oily fish as well as everything else. Kiwifruit & tomatoes, are a particularly good choice for softening stools and speeding up transit time in the bowel. And, with all the fibre, you might find that 1.5l water isn't enough, especially if your daughter is quite active. Could even be that she's getting too much fibre.... you don't mention her age.

With movicol to work effectively you need to drink a lot of water/squash etc. It works by attracting water to the bowel and softening it.

There is a good website called something like childhoodconstipation which may have some info.

She is 8yo, she drinks 1,5 litre, she takes kiwi, apples, greener bananas, pears, plum juice, now I also give her aloe juice from health food shop, I make only stewed dinners, make green salad including iceberg lettuce, carrots, tomatoes, a little of garlic, sometimes onion, she also eats cauliflower and broccoli and turnips. I also give her dried plums as well plum juice, I have bought beetroot juice, she sometmes takes that too. Also we love beetroot, I often make beetroot soup (it's our favorite) and beetroot salad is almost every day! She does not eat chips at all, I do not make any chips at home, she loves smashed potatoes, I often make at home, I stew chicken and beef, sometimes pork. I can't blame our choice of food, as all her stools are of very soft consistency like porridge... she just can't push it out, maybe she is still afraid, today we talked about everything what she is afraid of... hope she will feel more confident ... I put a Glycerol suppository just now, and she went to toilet and there was a good soft stool, but she still remains very sore around her tommy and is worried about school. I encourage her all time, she got a present on Sunday, I bought her what she wanted and she now sits on toilet herself even not asking me, I am proud of her. And yes she is very active girl... and she has never been nervous, now she has become nervous, distressed and easily becomes angry and embarassed.

Forgot to mention that I give her one tablespoon of olive oil every morning to lubricate the bowel...

Could be that the anxiety/stress you're describing (yours as well as hers) is part of the problem as well as a result of the problem. I know that IBS can flare up in people when they are stressed and how that is very painful. Repeated trips to doctors/hospital, swallowing endless sachets, having suppositories inserted, people checking your poo and the abdominal pain must be all quite a distressing experience. Is there any way you can calm it all down? Make things less tense? Less intervention?

compo · 20/04/2010 12:47

Agree with chil1234

you are understandably stressed up to the hilt

she must hate being so closely monitored by you and having suppositories etc put in

maybe you need to take a step back from it all

all the medicine must have played havoc with her system

Today I asked her to write down what she likes and what not at school and home, just let her think for a while and wite down, just to be aware what is that distressing her and talk about it. She did not mind, and we got closer and deeper to those small things psychologically which can play some role in all of this what we have been through. She then read them for me, and they were: she loves to be at home, she is afraid to tell her teacher that her tommy sores (and probably she is embarassed by toilet issues at school) because teacher some time ago since this started told her that they can't fix tommies and now this "can't fix tommies" stays in her mind all the time how much I try to tell her that teacher does understand because I already talked to her teacher and he was very understandable (as he has some tommy problems himself). Also she wrote she does not like when she is in toilet for longer, her classmates (girls) are shouting "hey when are you going out", but she has to sit for some while.
Nothing else, and I do not know, I will talk to her teacher again and let hm be aware of these things, it is the last I can do...

My 6 yo dd has difficulty in going to the poo. She has suffered from repeat urine infections since she was 3 but seems to be improving. however, this has caused problems with the whole 'going to toilet' issue. I think she is now suffering from repeat bacterial vaginosis.

We are under the care of the hospital but all they do is put her on AB's.

She dosn't like to ask to go to th toilet at school and will not ask the teahcr to go to the loo . She often comes home in dirty knickers. I have started to get her to sit on the loo at the same time every day and it's generally working (but with mishaps) She often wants me to come in with her! We play 'I spy' or barbies.

She has had worms a couple of tims too and she has told me sh eis scared to go becuase of them. Its so tricky!
