Is this right re treating a temperature spike

Hi. My DS (2) had a bronchoscopy today. We were told that one of the post procedure side effects can be a temperature. He was ok when we left hospital but by the time we got home his temp was 39.7. He had a max dose of calpol and an hour later it was up to 40.1.

I spoke to the hospital and they said to give nurofen but that if his temp spiked a second time I need to take him to our local a & e.

He had a max dose of nurofen and it's down to 38.5 but hasn't gone back to normal.

Can anyone tell me whether I need to be worried that the nurofen and calpol hasn't brought it down below 38? The dr said to take him to a & e if he had a second spike but what about if it doesn't drop in the first place? Also, since it has dropped, does it count as a spike as soon as it starts to rise again? I really want to avoid taking him to a&e cause he's had a terrible day already and was completely shattered. But I don't want to ignore symptoms I should be worrying about. Help!

Have just checked and it's dropped a bit more so think he's ok.
