I am so upset, no idea what has caused this, but have my suspicions.

Her diet is no different to her friends, some sweets , choc and fruit juice, but good brushing am and pm

BUT she takes lactulose and has for a year now 10mls at night just before brushing her teeth.

Should I change the time of the lactulose ( she swills it in her mouth before swallowing) Do you think this could be a cause of the problem?

Maybe I should ask for a change in her laxative?

That seems like a long time for a 5yo to need laxatives. Could she be weaned off them?

I would change the time of the lactulose, personally. Its very sweet, would defo brush teeth afterwards.

That said, my dd is now 6 and had three crowns and two fillings last year - the cause was almost entirely close fitting teeth which (to my amazement) I apparently should ahve been flossing, and wasn't. We always brushed twice a day, hardly any sweets, no lactulose: just soft enamel (genetic) and close fitting, otherwise very pretty teeth!

Don't blame yourself, or your dd - though do change something around the lactulose, IMO.

She has chronic constipation and without the lactulose would not go for up to a week
I have tried weaning it down, but last time we did, I ended up in A&E with her. it seems an ideal dose for her tbh

I have a feeling taht teeth shouldnt be brushed after something sweet but that it's better to have teh sweet thing afterwards. I think it's about the brushing making the sugar scratch the enamel and if you brush before the sweet thing then there is less plaque for the sugar to attach to.

We always brush dd's teeth before breakfast.

I am no dentist though.
