Is it done thing to ask a GP to fill in a health form for a child?

My kids live here but will be going to summer camp for a few week in the US. The camp requires that all children be checked out by a doctor who has to fill in and sign a form answering simple questions about the child's health and vaccination history.

This is routine in the USall doctors are expected to do this kind of thingbut I feel like if I asked my NHS GP to do this he'd think I was wasting his time. Am I right? And, if so, what are my alternatives? The kids don't have a doctor in the US, and anyway there won't be time for them to see a US doctor before they start camp.

Ask them - we have all sorts of forms to fill out it's not unusual and if it is something he can answer he won't mind. Sometimes they include daft questions like "Can you certify that this person is fit to bungee jump" (I don't know, I don't have a crystal ball although I don't have any information to suggest he is at more risk than anyone else )but if it is sensible and just asks for facts there shouldn't be a problem. He may have to charge a fee set my the practice though as it is not a service funded by the NHS.
