ds aged 3 bleeding when having a poo - please help

ds is 3. He's always gone for a poo every day, usually more than once. We've never had a problem. The other day he screamed while having a poo and when i wiped him there was quite a bit of blood. His poo was soft. I took him to gp who said it wasnt connected with virus he's just had, he hasn't had diarrohea so not connected there. Said it was bit of a mystery as poo was soft so shouldn't have caused any prob. He gave me some lactulose to make sure poo stays soft. Next day was the same only this time when i wiped him i could see what i assume were piles under his bottom. He has lumps and bumps everywhere. Went back to gp who examined him internally (nightmare!) and said everything seemed ok. Couldn't feel any piles but he said his finger would've pushed them out the way anyway so he wouldn't be able to feel any. Just told to keep up the lactulose. Ds now hasn't had a poo for 3 days, assume he's holding it in. I've just managed to insert a suppository given by gp which should act as a laxative. No luck yet. he keeps crying saying it really hurts. what do i do?

When you see bright red blood with a poo it is usually from bleeding right near the anus. He may have had a little tear when doing a constipated poo.
Ds1 had a fecal impaction last year. The way it went was first only having a poo every couple of days or so instead of every day, then very soft to watery poos. The pediatrician said that the poo starts to go around the impaction which causes it to be quite soft. We had to do an enema to clear the initial impaction and then he was on lactolose for a year to rehabilitate his colon. Apparently the impaction stretches out the colon and makes getting further impactions much more likely - which did happen when we tried to discontinue the lactolose after just a few months.

He was also 3 btw, which is evidently the most common age for this and it is also more frequent in boys.

I would try a child's enema or at the very least a glycerin suppository to see if you can get things going.

oops just saw you had tried a suppository. Time to go for the enema - but you will probably need someone to help you. It was a 2 person job with ds1.

He's just had a poo, been on toilet for almost half an hour. Once it starts to come out he will sit there until every last bit comes out so he's not storing everything. When i wiped him there was lumps and bumps and blood. No sign of tear on the outside, i'm still thinking its internal piles. Any ideas?

Sounds like the suppository did its job and you are probably right about the piles. Maybe some cream like anusol? and I would keep up the lactolose for a while.

Oh, and I would avoid dairy, bananas, applesauce and other constipating foods for a while.

He doesnt seem constipated though. Its soft when it comes out. He eats lots fruit/veg, good varied diet.
