Red itchy rash on son

I’m hoping someone might have some ideas because we’re honestly at the end of our tether.

My DS (4) has had recurring itchy bumps/rashes for months now. He currently has lots of small red itchy bumps scattered over his tummy, sides, back and arms, plus a red patch on his scalp.

We’ve been to the GP three times. Earlier in the year scabies was suggested, so since December we’ve treated ALL members of the household with ivermectin (adults) and permethrin cream three separate times and were incredibly strict with washing bedding, clothes, towels, soft furnishings etc. Despite all of that, the bumps have continued.

The spots are tiny red bumps rather than blisters, and they seem to come and go. He says they’re itchy but is otherwise well in himself.

We’ve considered things like papular urticaria/insect bite reactions, eczema, allergies, viral rashes, folliculitis and probably a hundred other things at this point.

Has anyone been through something similar? What was the eventual diagnosis, and did anything help?

PS my husband and I haven’t had any of these bumps so I really don’t think it has been scabies.

We have changed our washing powder to a complete non-bio/non-perfumed one for about 2 months now 😞 and his baths are pretty fragrance free/sensitive with an emollient in the bath. I just don’t really know what to do anymore! The doctors really don’t have any idea what it could be.

We’ve also been giving him antihistamines and using hydrocortisone which was prescribed by the doctor but it hasn’t really helped at all.
