Almost 4 year old behaviour deteriorated since being unwell - normal?

Hello! For context my almost 4 year old has a severe speech delay and awaiting an ASD assessment so not quite sure what’s “typical” in this regard.

My DS had an awful virus lasting around a week, with temps of 40, lethargy, off his food etc. Eventually he turned a corner bless him, but ever since then his behaviour has become almost unimaginable. Screaming, shouting, throwing things, banging his head etc. He kept saying “pain” but obviously couldn’t elaborate any futher than that, so we took him to get checked out and no signs of any lingering infection (ears, etc were all fine.) He also hasn’t mentioned pain since having recovered.

We’ve been very fortunate in that he rarely gets poorly like that and he’s mostly such a happy little boy, so it just feels like he a completely different child right now and was wondering if this is typical after a bad illness? He’s been exhausted obviously, so we had put it down to that but he’s back now in a good routine of eating / sleeping well and yet still really struggling and any little things sets him off, including noises that didn’t bother him previously so is needing his headphones a lot.

This is a theoretical condition, but not hugely well evidenced/researched as yet. It’s called PANS or PANDAS and is a neuropsychiatric syndrome following infection, particularly with Strep A infections. Your description would fit
