9 year old daughter with anorectal pain

Hello, I need help for my 9-year-old daughter.
After 4 days of constipation, when going to the bathroom, she strained strongly without succeeding in evacuating any stool. A terrible pain appeared in her anus/lower rectum. The pain did not go away. Went to the ER. They gave two enemas. Very painful. The problem is that the pain is so severe that my daughter cannot release the sphincter. Several other days passed with some other visits to the ER with enemas and no help. Stools partly released. My daughter has some sharp pain every day; sometimes every 15 minutes, sometimes every hour. A sharp pain like a knife in the anus. She clenches her buttocks during the pain. It eases a little. Then the pain starts again for about 15 seconds. And she cannot evacuate stools because it is too painful. The pain wakes her up even at night. She sleeps poorly and is weakened. Apparently, no fissure or hemorrhoid. It's not just constipation. All started when she strained first day. I would need some help. Thanks.

She needs to see a paediatrician who deals with constipation in children and are used to dealing with the subsequent psychological effects / fear of passing stool. A&E is not the right place (or “the ER” since it sounds like you’re in the USA). If you’re in America then then she should be able to get an appointment with her paediatrician on Monday. If you’re in the UK then you need to call her GP on Monday and they will see her promptly.

If it’s prostatic fugal also known as javelin bum i find sitting upright on something hard like a chair helps rather than lying down as it can last from about 10 -30 minutes, some people get it for a longer time but I find the earlier I catch it by sitting upright the sooner it goes ,I have read that hot baths and pain killers can work too . for me it’s particularly worse at night and you don’t always get warnings when it’s going to happen . I believe it has been linked to hormones so possibly look into that .
there’s quite a bit of information once you start looking ,it’s a case of trial and error as I don’t believe there’s any treatment available as it’s such a random ailment and don’t let anyone down play the pain because it’s the most painful thing i have ever experienced and I’ve given birth twice .

Can you have her soak in a warm bath and seeing if it’s less painful for her afterwards- even try pooping whilst in the tub?

I know it’s a bit gross to clean afterwards but the warm water might relax the sphincter and will also soften any very solid fecal matter. The reduction of pain when passing it should help stop her tensing up.

When my daughter was about 5/6 she got very constipated. After about a week of not opening her bowels and taking laxatives, she began leaking liquid fecal matter during the night.

She had so much pain that the doctor ended up prescribing her a muscle relaxant and advised putting Vaseline on her butthole before she got onto the toilet.
