Bowel movements

Hello!
Son is under the continence team, he has been for many years. He is still in a night time nappy at age 7 so I have been asked to produce bowel charts for 2 weeks and bladder/fluid intake for 3 days. I sent over the 3 days and have received a phone call saying that my son is constipated and needs to see a GP (I've made an appointment). This happened a couple of years ago as well and son refused the meds, he is asd and adhd, only drinks water and nothing else. The meds had to be mixed with water so he wouldn't drink it as he could taste them.
Continence lady said that that's why hes still wetting at night, because of the constipation.

What i'm confused with is that son has a bowel movement every day, sometimes twice daily. On the Bristol stool chart they're usually a 3, 4 or 5. Is this still constipation?? What experiences have you had with this please?

I have no idea and I hope someone will come along who does know.

But, surely these are questions for the medical professionals that your son is under?

Apologies I should have updated!
Saw the GP yesterday and after examining him and looking at the bowel charts, the doctor confirmed that hes not constipated.

When i asked, the woman from the continence team told me that you can still be constipated and poo every day.
