Significant drop on weight percentile

Just had our 27 month review with the health visitor, DD was weighed and she has gone from above the 75th percentile to the 25th percentile since her last review one year ago. The health visitor didn't seem overly concerned and suggested checking in again in 6 months just to keep track. is it normal to have that much of a drop though? It seems quite a big drop.
DD has always seemed quite petite, she's a snacker more than a meal eater and her diet is quite limited as she can be quite fussy but she has always been that way so it's nothing new.

Hi! Just wanted to come on and say that I have read in many places that toddlers do not put on weight as quickly after they turn 2.

I am also worried about my son's health (though we have other worries in addition to the weight) but I would generally see how she is doing in general and if she is happy and active, that's good!

Wobblyheart · 17/07/2025 14:29

But also, you can always take her to the GP for a second opinion for a peace of mind but most likely thing is that she is more active and eating less. Has she become a fussy eater by any chance? That what's happened to mine!

Height is more indicative of any issue like coeliac eg if no growth in height and dropping height gentiles

thismumneedssun · 17/07/2025 16:55

@Wobblyheartyes she's always been a picky eater but has become a lot worse since turning 2, she's also been 100mph since she started walking at 10 months she doesn't ever stop so I'm sure that's all it is but I was shocked at the drastic drop on the chart!

@cestlavielifewould not gaining weight be the only indication of coeliac? She seems otherwise healthy I think. she actually wouldn't let them measure her today but I will try to do that and check her chart

Not gaining height can indicate coeliac egg my dd failed to grow in height from age 2 . So she didn't move out of 12 to 18 months clothes for example was still wearing them age three
The weight was not so relevant
If your toddler is growing out of their clothes into next height range then weight centile may not be relevant
They just becoming taller and leaner

@cestlavielife sorry height is what I meant to type. she has always been around a size behind in clothing and has grown very slowly this last year, still comfortably in 18-24 months at 2 years 4 months. Feet have only grown half a size since she started walking also but again not sure if that can be normal, she is very petite.
I will have a read up, Thankyou

It is about following a line .
Try get good measurement maybe lying down and re measure in one month or three

My daughter didn't outgrow her 18-24 month clothes until 4! She dropped from the 25th to 1st percentile for height between 1 and 2 - She was in 9-12 month clothes for over a year and didn't outgrow them until 2

She has a restricted diet due to allergies but her paediatrician and dieticians happy with her. She was tested for coeliacs when she was having issues with wheat/gluten.

Over 12 months that's not necessarily a concerning drop especially as it coincides with increased activity, my daughter dropped just over 1 percentile between 6 and 12 months when she started moving but hadn't started eating. Review again in 6 months seems fair, did they check height too? I'd ask them to do so if they didn't. If the did you can use the NHS BMI calculator to determine their BMI percentile to see if they are a decent weight for their height. With 12 months between measurements you don't know when the drop happened, a dropped followed by staying on the 25th is different to still actively dropping which is where the repeat measurement comes in. When my daughter dropped height we had some extra measurements to check she had stabilised and wasn't still actively dropping height percentiles. You could weigh them yourself every 2 months to see if they are steadily gaining weight.

Have you had dietitian support for the limited diet? It might be worth seeing if you qualify for a referral to check there's nothing essential missing from their diet. My daughter needs a decent multivitamin due to her diet and has regular reviews with dietitians. Initially this was for allergy support but now she doesn't look to be outgrowing them it's switched to ensuring her safe diet is adequate. She's also a picky eater which adds complexity!

@SuperscientistYes that is true we can't know for certain when the drop happened, thinking about it she had a couple of months over winter where she had one illness after the other, sickness bug, the flu and then chicken pox, which obviously made the fussy eating much worse so it's possible that's when the drop was but I will keep a close eye on wether it has stabilised or not.
She wouldn't let them measure her height at her appointment and I've been unable to get a reliable measurement today when I've tried but I will keep trying so I can check that and think I will take her to the drop in weight clinic every few weeks just to keep a check on that for my own peace of mind too.
No, we haven't been offered any kind of dietitian support but I think I will enquire about it next week, even if they can just offer some guidance on how to get as much goodness into her as possible with the little amount she eats. I do already give her a multivitamin daily because of how fussy she is, I never know from one day to the next what she's actually going to eat.

I managed to get a height measurement today and she is still following her percentile line for that so that has slightly put my mind at rest. Her bmi calculation is sitting at the lower end of healthy weight but still within range. Thanks for all of your advice, I am just going to monitor her for now and see how she goes.

I had similar with my now nearly four year old however she was breastfed as a baby and comfort nursed 24/7 so was perhaps bigger than she would have been if she fed less frequently. She was wearing 18-24 month clothes until she was three (we got about 18 months out of them) then she had a big growth spurt. I’m short with a small frame and my husband is average height so dropping to 20th centile made much more sense to me than her sitting up on the 70th.
