3 year old spiking temps please help!!!

So I’m not really sure what to do, my 3 year old was poorly for a week last week with temperatures of 39 and up, we got seen twice by a doctor who was really thorough, couldn’t find any obvious signs of infection or anything, did a urine test in the doctors but didn’t get enough to send to the lab but again no sign of infection, she called the hospital and they said probably viral let it run its course, so we did, over the weekend she was fine, playing outside, went to nursery on Monday and then Tuesday night she was spiking temperatures of 39 again, went to the doctors Tuesday he said her throat was red but not really sure what’s going on, just to add my daughter is non verbal and developmentally delayed so cannot explain if she’s in pain, he prescribed antibiotics, she had 2 doses on Tuesday, 3 yesterday and one this morning and everytime she has the antibiotic her temp goes down and she’s up and playing but the minute it wears off she’s spiking temps again, the highest we had was 39.8!! Calpol and nurofen aren’t even touching the sides! We had the paramedics call and assess her and also had another doc from the hospital call at 3am to assess her, she’s only eaten grapes and a bit of cheese in the past 3 days and is only sipping her drink every couple of hours, we got told this was fine and the shivering, fast, rapid breathing etc was just her body fighting the infection.
Now my girl has pica and is clearly lacking something and she eats handfuls of mud, not just chew and spit out, EATS and SWALLOWS it, the weekend before she got ill the first time I caught her eating it in the garden and then the weekend she got better again (quite impossible to avoid) but I mentioned this to them because I’m worried she’s eaten something that’s made her poorly but they don’t seem to be worried and said about a blood test ‘once she better to not stress her out’ but she’s so unwell! My question is what do I do now?! Do I just ignore all of that advice and assessments and take her to a&e to demand further tests? Surely she shouldn’t still be spiking temperatures of nearly 40 by now???

Sorry for the long one and thank you for any advice I’m so afraid we’re missing something.

Follow your instincts and take her to A&E. If she’s been eating mud she could have a bacterial infection and the antibiotics she has are too generic and she needs something more specific and strong. If she’s not eating and drinking she could get dehydrated fast.

If you’re not happy with her progress it’s always your mum instincts telling you to do something. I’d rather be safe than sorry with a child.
