8 year old impacted stool

After many months of tummy problems my 8 year old was finally seen by GI consultant peadiatrician last week and had x rays and blood tests etc. Consultant rang me today to say she is indeed severely constipated; we already have some medication but she said we will need more and to wait with treatment until we have prescription for more. Naturally I have now read everything there is to read including on here and ERIC website and I'm not sure I understand our treatment plan. She's going to put this all in writing for us but I have asked her twice today and this is what she wants us to do:

  • two days of "cleanse" to be completed over the weekend; for this she told me to give senna and another stimulant laxative twice per day
  • laxido for unforseen future, dose to be confirmed and adjusted with further follow ups.

This seems different to what I read on ERIC page which suggests stool softeners to be first point of call. I worry that senna being stimulant is going to result in painful cramps without stool softener, surely all that backlog of stool is going to be rock solid by now? Or am I misunderstanding how these medicines work?
She's still having bowel movements but either small and hard or diarrhoea; no soiling. I'm aware that this is overflow from being so badly constipated. I can't even begin to describe how stressed and worried I'm for her; she's sensitive and anxious soul as it is my poor lamb.
We are also now back to dairy free diet (she was dairy free as a baby and toddler, non IgE cmpa) and this has helped a lot with tummy pains. We were initially referred due to suspected IBD (very high calprotectin and mucus in stool, plus frequent tummy pains and bowel movements as described above).

My DS had impacted colon and ended up in hospital. He had 10x sachets of movicol a day for 5 days to clear it and then filter down to 2 sachets a day for 6 months to give time for the nerves in that area to repair. He was also 8 at the time.

That sounds awful 😖 I hope it won't come to that. I hope your little one is doing better now.
I'm just concerned consultant doesn't want us to use laxido first as that seem to be the normal protocol. I have Image in my head of her writhing in agony and us rushing to hospital. I am up at 5 am for work tomorrow, I should be in bed two hours ago and I can't stop reading about poos!

I think you’re right to be concerned about pain from the stimulants without a softener. I’d join the Facebook group Movicol Mummies and ask for advice on there. I’d also ask the Consultant straight why they’re suggesting that regime and tell them you’re worried about pain from the stimulant causing the bowel to contract against hard stool.
For now though, go to bed! It’ll all be there tomorrow.

I am aware of variant regimes that use movicol and a stimulant laxative at the same time to achieve disimpaction, but I hadn't seen any that are only the stimulant.

My son (5 years) suffers from chronic idiopathic constipation.

He’s now on a maintenance dosage of movicol and a laxative. He was on 12 sachets of movicol a day at one point with 2 laxatives to clear the impaction . The initial clear out was horrendous. So have plenty of wet wipes and nappies on hand as it will just leak out .

Id also be concerned about her passing such large hard stools without the aid of a stool softener . The Laxido will help clear the impaction . Straight to the laxative seems a harsh and quick fix which will result in pain , and fear for the future of going the toilet so I’d question this too

Please tell her to not strain . She’ll risk an anal fissure which can be very unpleasant and painful.

Completely agree with you, a stimulant should not be started without at least 2-3 days of high dose softener first or it will cause pain and cramps on its own. You do realize you don't HAVE to do exactly what the consultant says. Paediatric movicol/Laxido can be purchased over the counter without a prescription and you could start giving her a few sachets a day for your own peace of mind before beginning the stimulant from the consultant. Do what you think is best for your child. Doctors don't always know everything.

We tried with just senna and lactose (advised by gp) and it was brutal, she started withholding and was in awful pain because it was still hard and being forced out. DEFINITELY ask for movicol, it’s been a life saver for us.

Both of my DDs have had constipation problems. We used so much laxido and movicol over the years. Personally OP I would buy some laxido and also some paediatric glycerine suppositories and use them before the cleanse.

Also, I totally agree that stimulants are not the way to go and seems an unusual choice - softeners is the usual dis impaction regime.

Thank you all. I will definitely speak to ERIC and air my concerns with consultant once prescription and letter comes through. We already have laxidoo, senna and picolax but having reviewed her x ray consultant said she will need more. Initial plan was one day senna with picolax followed by long term laxidoo but she now wants us to do two days of senna and picolax over the weekend. From what I gather this is protocol used for bowel preparation before colonoscopy and my sister in law told me she was in absolute agony with this treatment and she wasn't even constipated! At the same time I acknowledge that there's a reason why our doctor is consultant peadiatrician specialising in gastroenterology and I'm not so perhaps she has her reasons for this regime but I will definitely need to know more. I know she wants us to do it when she's off school for minimal disruption but I'm really uneasy about this. I need to make it very clear to her how anxious and sensitive my kid is; she's only just started overcoming selective mutism for example. If this goes wrong I'm sure we're in for a lot more bowel related troubles in the future.

I discussed my concerns with gastro nurses and they reassured me so I proceeded with regime as prescribed; started this morning. She drank first sachet with minimal fuss and swallowed tablets with a little bribery. She's had some tummy pain but nothing major, Calpol and heat pack helped. After couple of hours she did one big soft yellow poo and been since 4 times passing just water. She's drinking loads of squash, lemonade, apple juice and home made chicken broth. She's had 2nd dose of picolax and senna over 3 hrs ago and still no action. WHERE IS ALL THE POO??!!😭 I'm properly scared that it hasn't worked or that she'll have accident at night. I'm keeping her up just in case for a little while longer. Surely what we had couldn't be it???! I asked nurse if we get to water stage on the first day shall we stop and she said absolutely not, we need to complete two days so we will continue but God it's stressful.

I know it’s late now, but for overnight could you try her with pull ups/ dry nights? It might reduce some of the anxiety about accidents.

I know you said she’s very sensitive, so have a chat with her letting her know that if she has any accidents it’s okay, it can happen with the treatment and to let you know so you can sort it out (I’m sure you’ve already done something like this)

Thank you both. She firmly refused pull ups but she roused herself around midnight for two more yellow watery bouts accompanied by quite a bit of cramps; it was soul destroying. This morning she woke up right as rain asking when is she getting her medicine again! I was expecting a battle but she took picolax again with no major problems and swallowed senna tablets. She again had more watery motions with less pain than yesterday but I'm at loss as to why is she not passing more substantial stools still. I think it's just overflow and main blockage is still there, I'm dreading what next course of action might be! Other half wants to feed her some sweetcorn and see if it passes through; i don't think it's particularly scientific method. I'm mentally drained from this, ended up taking dog for an hour and a half walk just to get away from it all for a while.

Well we have completed the process but I'm not confident it was successful. I will wait a day or so for her first "post cleanse" poop and contact hospital. She started yesterday on maintenance laxido and picosulphate and not pooped yet. I'm not panicking yet because there shouldn't be much there after two days of picolax but I felt a bit uneasy about sending her to school. We have intimate care plan in place for school and she had no problem making it in time even with explosive diarrhoea at home so I'm hoping she'll be OK. She has a school trip tomorrow though and I'd really like her to have a poo at home before that!

How did school go for your DD today? Just reading your update made me stressed for you. I hope she passes something substantial tonight so you don't need to worry about her tomorrow. I do think your DH's idea about sweetcorn is worth a try too.

Still nothing 😬 She said she didn't feel the need, she's had sweetcorn on Sunday and it passed through within hours but I'm told in theory it could make it's way around the blockage but it's certainly hopeful. She's had almost 2l of drink yesterday, we've been trying toilet 30 mins after meals and still nothing. Still hoping she might go this morning before school. Thank you for thinking of us x

WE HAD A POO!!! Sorry for shouting but I'm so relieved 😅 She went at school before they set off for their trip and from her description it sounds healthy so fingers crossed she will stay regular from now on. I can't believe how worked up one can be about bowel movements 🤣

Thank you to everyone who commented! So far she's doing good on 2 sachets of laxido and 2.5ml sodium picosulphate per day; she's going 2-3 times per day without any straining. Fingers crossed we can stay on that and e can wean her off at some point in the future x
