Split skin and red

I am very worried about my DS2 he is 9 months old and for the last 3 months been battling with sore nappy area. I do exactly as I did with DS1. We do Elimination communication. So I offer him the toilet during nappy changes which I do every 2 hours. And when he wakes from nap / morning and before nap / bedtime. Since starting solids he doesn't do No.2 in the nappies and goes once a day sometimes twice and we normally 9/10 times catch those in the toilet. No issues with bowel movements.
But since we stopped changing nappy at night time he has developed soreness on his front around the hip crease and around/on his genitals.
I was managing it and seemed to be getting better with sudacream. But last few baths i noticed he was really grabbing and pulling down there. And last night after his bath and applying cream I saw the sack folds actually split open. Not bleeding. But it made me wince and feel quiet nauseated. It looks so sore and pink around the area.

I dry the area very well and apply barrier cream. I offer 5mins nappy free time in the day and aftwr bath time. Could it be the nappies? Do I need to change him at night time. He wakes up with a very full nappy in the morning - no.1 only. I use the same brand as DS1 Mamia and he is in 4+ as he is a big baby.
I will change nappy brand and change him at night time. He wakes up twice for a feed still. Normally he nurses for 10mins and then goes straight back to sleep.

Anyone have any other suggestions or ideas what it could be. Or anything to help heal the split skin. My DH said it probably burns as it doesn't bleed at all. His bath waterbis always warm not hot.
We waah the nappy area with a mild unfragranced baby soap.

I feel so bad for him.

Sounds like it could be a fungal infection, he needs to see a GP.

Do you use wetwipes? And check the soap (and maybe even the sudocreme?)

My youngest (and my ex) used to get sore, red, splitting and bleeding skin from the preservative Methylisothiazolinone - literally just a blob of bubble bath in his bath and his skin would go pink, a couple of nappy changes with the wrong wipes and his skin would be bleeding. Once I was sure they'd changed the recipe but not the ingredients on the pack because the effect was so immediate.

Edit - or yes, does also sound like fungal is possible.

We use wet wipes when out and about. We use Water Wipes brand. At home just the mild soap and water. Then a fresh flannel to pat dry. We did this with DS1 and never had and issue.

Oh wow. What did you use instead that helped?

I have to use Boots bamboo nappies at night (Little Stories); just came back from a holiday using Pampers because we couldn't take enough bamboo overs and nappy rash recurred. I think Pampers trap the moisture next to the skin with whatever makes them ultra absorbent.

I second the recommendation for using cotton wool and water for changes at home.

I was just very careful with the wipes we bought (although the time he had a reaction was a shock - but switching to another brand fixed it. And as he got older, bubble bath, soap, shampoo etc. Now he's an older child, he doesn't bleed immediately if he uses the wrong soap, but his hands go red and feel a bit burny - if he keeps using it then they do start to crack.

Water wipes don't have it they say, but check other products (and the fungal issue if not)

Edit - and yes, his older brother never had this issue, neither have I, but his dad had had issues for a while, and once we removed all the products with this preservative in from the house, his skin issues cleared up too (sore scalp, hands, err. nethers from toilet wipes)

Yes will try a different nappy. We only ever used Mamia but I think he pees so much at night that maybe it os not effective and moisture is on his skin and he sleeps 12 hours.
Such a pain as we are due to go on holiday in 5 days aghhhhhh

Just wanted to update everyone and if anyone has this happenn to them.

Went to the Gp who confirmed it was fungal but said it was mild and that they has "seen worse". Sorry but the lacerations in the creases if the scrotum was horredous and I Sony know how anyone xouod let it get worse.

Anyway given canestan to be applied 3x a day and steroid x2. Told to apply canestan then wait 15mins then apply steroid. Blooming pain and has made leaving the house rather difficult.

Spoke to famioy pharmacist who said there is a connection between recent illness snd viral rashes occurring. And ended up meeting a Mum who said everytime her LO is unwell at the very end they get nappy rash. How bizarre! Nothing I did which is what I was winding myself up about.

Symptoms are improving slowly and just using cetraban to wash yhr nappy area. The lacerations are looking lioe they are healing up and the redness has calmed down. I hope once the course is finished that will be the end of that.

Update! Yea still ongoing. 7 Go apts and 1 paediateician apt later. Swabs all fine no fungak / no Baacterial and no viral! No one is helping infact they are now saying it is all in his head!!!!! He is 11 mo tha old! They are saying he wants to scratch as a bad habit. Like people who have eczema who create borrows?! What the heck! I am at a loss no one wants to help and he is still scratching like he has fleas!!!! Napoy free time is still impossible. And I have to restrain him for nappy changes and bath time so the cuts and lacerations all healed up but still nearly 3 months and atill suffering.
I changed nappy brands 4 times. I tried all sorts of natural and pharmcy based creams. They just make him go crazy. I never use water wipes ans only use water and cetraban for cleaning.
What do I do? Where do I go? 😭
