13 Month Old Uncontrollable Screaming

My 13 month old has had a few instances of uncontrollable crying where he screams and thrashes around for up to 45 minutes at a time. It can happen at any time of day including first thing in the morning, after his nap and before bed. He's not unwell but does have enlarged adenoids and a bit of glue ear, we're considering an adenoidectomy in the next few months. Has anyone else experienced this in a baby this old? It would be normal in a newborn but it doesn't feel right at this age. Could it all just be the glue ear and fluid or is it something like autistic meltdowns? He is behind on speaking and pointing but seems pretty engaged and socially aware and his eating and sleeping are generally good unless he's ill. I don't think he's in pain, usually he eventually gets tired or distracted and stops and is completely normal again. Does anyone have any advice?

Sounds a bit distressing for all of you. My DC1 used to get very upset after his nap will we worked out that if we gave him water he drank it and calmed down.

If you think he’s not in pain, have you tried giving him pain relief a couple of hours before bedtime to see if he still does crumble like this before bedtime?

Thank you for your reply, I'll try the water, I do think he gets dehydrated because he has to mouth breathe. And the issue with the pain relief is that the crying doesn't happen all the time, maybe 2-3 times a week so it would be hard to judge.

Hi, Bumping this as we took him to the health visitor who observed and said that the meltdowns may be an early sign of ADHD. He's still only 14 months but does anyone who has an older child with diagnosed ADHD have any experience with their child being like this as a baby? Did you find anything that worked to calm them?
