Looking for advice on son's blood test results

Hi,

Any advice would be much appreciated.

I took DS (age 6 at the time) to docs before Christmas as I was concerned about ongoing tiredness, Although he does have some issues with getting enough sleep during term time, I thought I should get him checked out as he often looks shattered, dark circles under eyes and pale (though he is pretty pale skinned anyway).

Docs informed me bloods were ok, towards lower end of iron range but nowhere near anaemic, so suggested dietary improvements.

However, have requested full results and a number of things have been flagged up. I just want to check there's nothing I should be following up on?

These are the resuts I'm concerned about.

Tissu transglutaminase IgG lev - (SIO) - 1.normal 3.1 U/ml 0.00 - 10.00U/ml
To exclude coeliac disease with confidence this child must have been
eating a normal gluten containing diet for at least 6-8 weeks prior
to this blood test. If you still have concerns please make a
paediatric outpatient referral for expert advice.

23-Dec-2024 ! IgA - (SLR) - OK (normal anti-tTG) 0.35 g/L 0.50 - 2.40g/L

23-Dec-2024 Serum ferritin - (SLR) - 3.speak routine 26 ug/L 15.00 - 300.00ug/L

23-Dec-2024 Tissu transglutaminase IgA lev - (MW) - 1.normal <1.0 U/ml 0.00 - 10.00U/ml
To exclude coeliac disease with confidence this child must have a
normal IgA level and must have been eating a normal gluten
containing diet for at least 6-8 weeks prior to this blood test. If
you still have concerns please make a paediatric outpatient referral
for expert advice.

! Monocyte count 0.6 109/L 0.70 - 1.50109/L
! Eosinophil count 0.2 109/L 0.30 - 0.80109/L

! Serum albumin 49 g/L 35.00 - 45.00g/L

I did wonder whether Vit D levels might be an issue - I was diagnosed with a deficiency myself a few years back and strongly suspect this would have been the case when I was pregnant (and I didn't take the Vit D supplement during pregnancy). Plus he's pale skinned so we're pretty vigilant about sunscreen. He is now taking a multivitamin with Vit D but I know myself if he's properly deficient that will barey touch the sides.

any ideas??

Low iron, tiredness and the low iga etc actually does indicate coeliacs disease. Does he have any dizziness, eye disturbance, bowel problems, reflux, nausea?

Thank you. No, not really. Bowels occasionally a bit sluggish if he’s really overdone it on bready things, but he’s usually pretty regular!

Isolated low IgA is hard to interpret, may or may not mean anything for you. Probably best to just discuss everything together at next GP consultation.

Generally testing of vitamin D levels is avoided unless there are specific features of deficiency. It would be unusual in a child with light skin tone who spends a reasonable amount of time outside and has it supplemented orally anyway.

yes, ferritin within normal range but they told me it was a mild iron deficiency, I presume because it was towards the bottom of a big range 🤷‍♀️
