Trimethoprim WORRIED MUM

hi there
at my 36 week scan my baby was diagnosed with kidney dilation.
they induced me at 39 weeks.
he was born early Sept.
since birth has been Trimethoprim.
starting doge was half a ml.
last week GP put this up to 20ml a day. 10ml in AM and 10ml PM.
This has knocked my 8 week old out. he is sleepy. he is having less feeds. I have raised this concern to my GP. she said sleepiness is not a side effect.

has any one had any experience with this ? I do not want him to get an infection. but if he is to drowsy to feed surly he will get infection from dehydration.

I am a first time mum and don't want to 'over react' but I need to speak for my baby

How much does he weigh in kg? Are you absolutely sure it was 20ml and not 20mg?

He is baby it is best to speak up

20ml sounds wrong. Dose for my 8 year old was 5ml a day of trimethoprim, for similar reasons - prophylaxis against infection.

The BNF advice is

Child 6 weeks–5 months
2 mg/kg once daily, dose to be taken at night, alternatively 12.5 mg once daily, dose to be taken at night.

So it will depend on how many mg per ml. 20ml does sound a lot though.

on the bottle GP percribes says 10ml in morning and 10ml at night.
it's 50mg per 5ml.
so his getting 200mg a day

I have said to GP this is a high dose she said it's to his weight.

prophylaxis dose - I am under the impression that not giving him any will give him a kidney infection bit giving him so much to make him drowsy and not feed - will also give him an infection.

So he's roughly 5.3kg. If the GP is following the BNF guide of 2mg per kg per day he would need 10.6mg, not 200mg.

You can check with pharmacy as well

I'd speak with a pharmacist as they tend to be more knowledgeable about dosages etc.

I would call the dispensing pharmacy and ask to speak to the pharmacist to check dose. GPs get medication dosages wrong more times than you realise. Your pharmacist is best placed to give you advice re this.

That does sound like a shockingly high dose for prophylaxis

I'd go back to the dispensing pharmacy first and check with the pharmacist specifically, not a member of the counter staff. Ask if what was dispensed was correct and then if yes, if what was prescribed was correct by their reckoning.

hoping my baby wakes up enough for his next feed - this morning dose should be wearing off soon hopefully

Has the doctor muddled up mg and mls? That seems a much higher dose than a baby would normally have? Definitely speak to a pharmacist.

Does the label say 10mls?

Just edited to say it’s probably the original bottle so won’t. Thats a hell of a jump and doesn’t sound right.

contacted pharmacy they belive its a high dose
called GP advised to go to emergency department
I will call for an appointment in the morning
i havnt gave his PM dose
he woke up 6pm ish back to himself

I was going to say that sounds high too but you've got help. DD had reflux as a child and was on low dose at night too

GP gave wrong dose.
they said it is there f*ck up.
he is unharmed thankfully and is now on 2.4mls.
