Can anyone decipher this vaccination record?

Sorry this is so boring but I am confused. I am filling in a form which requires me to tick various vaccinations, but they don’t seem to match up with the ones in the red book. Maybe it’s different terms for the same thing? I have tried to google this but couldn’t confirm. I have included some pictures.

For example at eight weeks DD had a Men B, not mentioned on form, but the form requests a PCV, which I can’t see in the red book.

Then at 12 weeks she did have a PCV, but the form doesn’t ask for that at 12 weeks, it asks for Men C which I can’t see in the red book. At 16 weeks the form asks about a PCV second dose, but red book says Men B.

Very confusing. But from looking at the red book pages, they seem to be and look correct, if I remember rightly from my 3 DS' red books, as there is 100% a Men B on the 8 weeks, that isn't on the form you're filling out like you said. Where are the forms from/for (ie school, hospital?!) if you don't mind me asking? Misprint or confusion on them? I may be reading or getting it wrong but that's my view on it x

Either way. If she is up to date and has had every jab without you refusing any, I'd inform them that she is 100% up to date but the forms aren't matching the jabs sections in the red book x

I would just tick the ones she's had and write the correct date in, without worrying if it doesn't match up to the recommended ages on the form. Or if it's for something where it would really make a difference, call them and ask for advice.

I've had different ages given for vaccinations when we moved house. And once dd was off sick on a day when the HPV vaccine was administered at school, and when she went to get a catch up at the clinic only 2 weeks later, it had been moved from a two-dose to a one-dose vaccine!

Thank you so much for the replies, now that I know it’s not just me missing something obvious I feel fine doing as you suggest and just filling in what she has had. The important thing is the red book doesn’t seem to have any gaps so she is hopefully on track. The forms are for a preschool, so I think maybe they are just slightly out of sync with the red book then.

I had the same when filling in a form for nursery. I brought in DD's red book and explained the problem and showed the red book to prove she'd had every jab. They were happy with that and just said tick every box. I recommend doing the same, bring your child's red book and go and speak to them.

The vaccines your child has had in the red book are in line with the current child immunisation schedule for England. I will attach a link for your information.

The layout and vaccines in the form is an older immunisation schedule.

The next vaccines will be due when your child is 3 years and 4 months, this will be the second dose of MMR and booster of diphtheria tetanus polio and pertussis. I hope this helps.
