Scarlet fever secondary skin rash

Hi

looking for some advice. My LO diagnosed with scarlet fever. Literally had no symptoms other than rash appearing which looked typical of scarlet fever. Given penicillin which was taken for one week and DH and I agreed it wasn’t getting any better. spoke to doctor again who didn’t see LO and going by pics said it appeared to be a secondary skin infection. Course did Dr Google and can’t seem to find anything relevant. for some context we’ve had some skin issues that have worsened over time and particularly in the last few months. like a nappy rash that will not go away, was examined to be told that it could be eczema / likely not psoriasis and creams like metanium, sudocrem etc won’t clear it. Nor will Dermol or other ointments. Skin sample came back negative for fungal infection. It has definitely gotten worse with this sickness. Feel very sorry and helpless for my LO 😟
have that mum guilt where you feel it could be something ‘more’ or underlying. He’s always had stubborn cradle cap (or so I thought) but pretty sure it’s just dandruff and fungal. Other than that withholds a lot in terms of bowel movements. No other issues. Anyone any similar experiences or ideas, could be gut related? Can a blood test reveal anything or will I need to do a food diary? I dread the thoughts tbh, his eating is difficult as it is without having to start removing things.

the doctor didn’t mention this at all, which surprised me. I’ll raise it with him later. Also don’t want to come across here like I disregard doctors’ diagnoses etc but I’m a little fed up in doctors just seeming to treat on immediate presentation, whereas I feel this could be a plethora of things that might require more investigation. No exaggeration… metanium, cetraban, epaderm, Dermol, sudocrem, timodene…. None of them have cleared this ‘nappy’ rash. It’s like bright red angry and sometimes raw
