Why is my toddler vomiting?

Usual disclaimer that yes, we have contacted the doctor and are waiting for an appointment. We are also waiting for a private paediatrician allergy specialist. Hoping the hive mind can help spot a pattern that I am missing, though.

For the past week, DD (14 months) has been sick once or twice in the evening. Previously she had a cough and would vomit when she was all phlegm-y, but the cough has pretty much gone.

  • she is sick without warning, no gagging or retching
  • it is generally liquidy, regardless of when she last ate
  • it is copious - not just a little spit up - requiring full outfit change including sleeping bag, sleepsuit and vest, and sometimes a hair wash
  • sometimes it is before milk, sometimes during, sometimes after
  • sometimes when she is still in our arms (ie not lying flat), sometimes when she is in bed
  • it doesn’t seem to bother her afterwards - the other day she slept through it even though the noise woke me up on the monitor.

I cannot see a pattern at all. It’s never the same time, never the same point after food/milk. It is always after her bath, so we didn’t use bubbles last night in case it was her swallowing them, but that made no difference. I know allergens can take a while to leave the body, and we are keeping a food diary in advance of our medical appointments. It is (so far) only at bedtime, not a reaction to all meals/milk.

Background - she had silent reflux as a baby. I suspected lactose intolerance/CMPA or similar. GP referred for blood test for “all main food allergens” and came back negative. In general she only vomits either with a cough, or when she had tonsillitis. She’s not historically been a “sicky” baby.

sorry, it’s long! Just hoping there is something super obvious that DH and I are missing.

  1. They can't test for intolerances,at least I don't think. Even if they reassured you about lactose allergy, she could still have an intolerance
  2. If vomiting followed viral infection and this time she had a cough, maybe it's connected to that virus that caused the cough. Did you test for covid? Sometimes covid can linger in the gut and cause GI symptoms. I assume other viruses can do this too. Thanks @ForRoseExpert - stupidly I hadn’t even considered that she could still be intolerant. This started after we tried to increase her cow’s milk/decrease her formula, so I wonder if that’s it. She has always consumed vast quantities of yoghurt and cheese, but does get a bit spotty around her mouth after meals. We were always advised it was dribble rash but I’ve never been convinced.

We have cut out the cow’s milk for now (but kept other dairy like yoghurt) and will see if she settles down. Thanks for the website link - v interesting reading! I feel woefully uninformed about nutrition for her, really.
