Sleep disorders - 4yo

DS is 4.5 and as a bit of back history, has had sleeping issues since birth. He has never slept through at night and wakes anything from 3 - 8 times. We hoped this would improve as he grew but he still sleeps like a newborn! I have asked GP for advice but always feel dismissed, but I dont think its normal for a child his age to consistently wake every night, without exception. He is in good health, he watches less than 1hr of TV a day and never before bed, we do a bedtime routine of story, teeth toilet etc. Over the years we have tried variations of white noises, night light vs no light, own bed vs bed, weighted blankets, aroma sprays. He sometimes is a mouth breather and still has an obvious tongue tie, and I feel his tonsils look large ( dentist said yes v large, GP said normal) as ive heard these things can cause issues. We are now in the nightmare phase so when he is waking now, he is screaming the house down waking his siblings so its all amplified a bit right now. Many will say, oh its a phase at this age, but aside from the volume he screams at right now, its been the same since we brought him home from hospital! Anyone had similar? Did anything help!?

In regards to the large tonsils, if this is the case he may also have large adenoids. Have you ever watched him sleep? Apart from the mouth breathing, is there snoring? (Even mildly) And does he ever waken with a stopped breath? These are things to watch for with sleep apnea. If you can get any evidence of these things on video and show a GP they could refer for adenoid removal.
The nightmares, are there any anxieties shown throughout the day? Fears or worries? Does he remember them and is able to relay the nature of the nightmare to you? Are you sure they're not night terrors since there's so much screaming? Does he seem frozen or unable to recognize you?

He sometimes snores but not consistently, and doesnt pause breath. I know what to look for as his dad has sleep apnea which always panics me! He a generally happy kid, not anxious or worried and some nights he does recall the dream ( usually monster related) and other nights he is looking straight through me and is not fully awake. Theres a pattern if sleep walking / talking in his dad side family...
