Phimosis 4 year old - behaviour

My son 4 has confirmed tight foreskin (phimosis) for 2 months and he does feel urge to wee a lot - at first 30-40 times a day and 1-2 hours awake at night to run back and forth to toilet, then we saw the doctor and started steroid cream 4 weeks ago and he goes half of the times than before and wakes once every night to wee and mostly goes back to sleep. He can’t go to bed until late due to peeing and it does make him slightly uncomfortable. Doing all we were told by doctor, but he isn’t cooperating well and due to see a doctor this week again to see the progress.

But the big issue is -

His behaviour coincidentally got worse around the time the issues started (which he was treated for uti (wrongly) and has gone from challenging behaviour to unbearable every day. He is defiant child, but he now has awful 30 min to an hour outbursts, anger and grunting for saying no to something. It would be normally resistance, but not a complete meltdown and even getting aggressive. I’m at my wits end, even if I try to be calm and understanding, his daily outbursts have emptied me out (he does not even react to anyone when he is in that moment)

Question is - Is his phimosis affecting his behaviour or it’s a deeper issue (I’m starting to think it’s adhd, which I have suspected for over a year now) , but never had such big tantrums as for past 2 months.

If he's uncomfortable then of course it can exacerbate any behavioural issues. Similar to that a lot of people get short tempered if they're in pain.His condition effects his sleep, which won't help either.

Have they said he should have an operation?

My gosh this brings back memories. My then 4 year old was diagnosed with phimosis xerotica. We went through using steroid creams, he stopped drinking water so he didn't have to urinate, going to pee loads of times, constant uti and he finally had surgery. He's in a lot of pain so go easy but obviously do not accept bad behavior. What have the doctors said?

My 4 yo had circumcision last year due to phimosis and to be honest the circumcision only helped him with his potty training and some of his grumpiness. He was 3 back then so wasn't as articulate as he is today but I would say he was less grumpy but Behaviour wise, this year we found out that he has sensory issues which causes the difficult behaviour such as being hyper, not listening, crying and lashing out. He isn't autistic and his paediatrician doesn't think he has adhd either.
