Please help me identify rash?

I've quite literally tried every cream and remedy, so I'm not looking for suggestions on that front. If you've tried it so have I but it didn't work. Visited the pediatrician several times and ped dermatologists and allergist as well. Nobody knows what this is. The rash goes away and comes back as suddenly as it disappears. We have discarded acid poop, fungus, yeast, diaper brands, cloth diapers, any sort of irritation, etc. Air time doesn't make a difference either. He just sometimes gets it and it's stubborn and then it goes away. Any ideas of what type of rash it could be? I'm literally asking here because the doctors are out of ideas at this point and have told me it'll go away as he grows.

Hi OP. My little one gets a rash like that when she's teething sometimes. Not every time, but when its particularly bad she seems to! Is there any link between flare ups and teething?

Thanks for your reply. That's what we originally thought, that somehow teething made his poop acidic, but he has all his teeth since 18 months (20 months now) We are waiting for the second molars but those are much later, right?. It doesn't look like there's inflammation in his gums or anything...

My first DC had all his teeth by 22 months so I think it can really really vary on timescales.
Is the rash bothering him much?
It does look identical to what my DD gets now she's teething but it doesn't seem to bother her much. I just avoid wiping and dab it dry with a clean flannel after each change. And I tried all the fancy creams that helped DC1 but the only thing that works for DD is good old sudo! It's crazy how different they all are.

It does bother him quite a bit. He complains and cries while grabbing the area. It doesn't itch I don't think (?) I haven't seen him scratching a ton. Poor thing screams when I wash him and sometimes (I assume) it burns so much he doesn't want to bathe. Contact with water or soap or even applying a cream makes him scream and cry.

It's almost like it is a burn, you know? At least he acts like that.... Like when you get sunburn and it really hurts to the touch. Am I making any sense? ...
